Dosage form, composition

The agent is released in liquid dosage form. A medicinal suspension of white color without a pronounced odor. Drops are packaged in polyethylene tubes or bottles with convenient droppers. The original cardboard box contains one bottle (5 ml) with a detailed description of all characteristics.
Tobradex ophthalmic drops contain two main chemicals:
    glucocorticoid dexamethasone;
    antibiotic tobramycin.
It is these active ingredients that endow the drug with a therapeutic effect.
It also contains several additional ingredients. Auxiliary components help to increase the shelf life, increase or decrease the pharmacodynamics and pharmacokinetics of the drug, as well as significantly improve other indicators of drops.

Mechanism of action

The therapeutic effect is achieved through two active ingredients:
    Dexamethasone is a hormone that has anti-inflammatory, immunosuppressive, anti-allergic effects. Glucocorticosteroid activates processes that strengthen the protective membranes of cells and suppress inflammation. Antiexudative effect is a unique property of GCS, which is aimed at eliminating vascular permeability and removing fluid from tissues.
    Tobramycin is a new generation broad-spectrum antibiotic. Its main task is to influence the growth and reproduction of bacteria. First of all, the antibacterial component is active against staphylococci and streptococci. The destructive ability to other pathogenic microorganisms has also been proven.
When used topically, Tobradex drops have low systemic absorption. Mainly excreted from the human body by the kidneys. The time during which the concentration of the drug is halved (half-life) is 3-4 hours.

Indications for appointment

Ophthalmic pathologies of an inflammatory nature are the main indications for the use of Tobradex eye drops indicated in the instructions for use. The medication is prescribed in the following cases:
    conjunctivitis - inflammation of the conjunctiva;
    blepharitis is an inflammatory process that affects the edges of the eyelids;
    blepharoconjunctivitis - inflammation of the edges of the eyelids, accompanied by damage to the conjunctiva;
    iridocyclitis (anterior uveitis) - a combined inflammation of the iris and ciliary body;
    bacterial keratitis - acute inflammation of the cornea;
    keratoconjunctivitis - inflammation of the cornea and conjunctiva.
Drops are often used for various eye injuries and burns, as well as for the prevention of eye infections after ophthalmic surgery.

Dosage regimen

The instructions for Tobradex eye drops indicate the following standard dosage regimen: 1-2 drops into the affected eye every 4-6 hours. For the first day of illness, it is allowed to reduce the frequency of use to 2 hours. As the condition improves, the interval is increased. Treatment is carried out until all symptoms of the disease disappear. To avoid relapse, therapy is continued for several more days.
After ophthalmic surgery, 1 drop is shown up to 4 times a day into the conjunctivitis sac of the operated eye. The duration of use should not exceed 3 weeks. This approach will help to avoid the development of complications in the form of inflammatory processes. Therapy can be started the day before surgery.

List of contraindications

The high susceptibility of the body to at least one component of the medication is the most obvious contraindication. Additionally, Tobradex eye drops cannot be used for the following conditions and diseases:
    children under 12 years old;
    period of breastfeeding;
    purulent and fungal lesions of the organ of vision;
    eye diseases of a viral nature (viral keratitis, herpes simplex - chickenpox and herpes simplex);
    mycobacteriosis.
It is prescribed with caution to women in position, as well as to patients with glaucoma, thinning of the cornea.

Possible consequences, overdose

Against the background of treatment, pain, sting, burning in the eyes often appear. Erythema of the eyelids, keratitis, edema, decreased visual acuity, glaucoma, cataracts, dry eye syndrome, lacrimation, photophobia are rare side effects, the frequency of which is not precisely known.
With prolonged use of Tobradex drops, secondary infections, non-healing sores on the cornea, and purulent discharge are sometimes observed. These side effects are due to prolonged exposure to dexamethasone (GCS), which significantly reduces the body's defenses. An increase in side effects indicates a possible overdose. If you suspect an overdose, you should contact an ophthalmologist who will adjust the therapy.
The interaction of Tobradex with other drugs has not been studied, because drops have low systemic absorption.

Special recommendations

When using Tobradex drops for treatment, the following instructions should be considered:
    the drug is intended only for local use, it is strictly forbidden to use drops inside;
    when using the medication for more than 2 weeks, monitoring of the state of the cornea is required;
    the interval between the use of drops and other local ophthalmic agents should be at least 20 minutes;
    before instillation, the contact device must be removed, you can put it on after 20-30 minutes;
    the duration of therapy for children under 18 years of age should not exceed 7 days;
    contact of the dropper bottle with the mucous membrane of the eyes and other surfaces should be avoided;
    after each use, the bottle with the medicinal solution is tightly closed.
In addition, if there is a decrease in vision clarity immediately after instillation, then you should refrain from driving a car and work that requires precision. You can get behind the wheel after full restoration of vision.

Reviews

Dmitry, 28 years old
Quite recently I had to go to an ophthalmologist, because the right eye was very watery and painful. It turned out that he was hit by a small chip. The doctor removed it and prescribed Tobradex so that infectious inflammation did not join. The treatment lasted a week. I cannot assess the result, since I used the medication for preventive purposes. The only thing I can say is that there were no unpleasant reactions. Although the instructions contain many possible negative consequences. Yes, the drops are very strong, because they contain an antibiotic and a hormone. Therefore, I do not advise you to use them yourself without the recommendation of a specialist.

Drops Tobradex were prescribed by an ophthalmologist to his son for the treatment of conjunctivitis. I bought it, read the instructions for use and was scared, since it contains a powerful hormone, and children are contraindicated. At that time, the son was 7 years old. At their own risk, they started treatment. Dropped 1 drop 3 times a day. The drug helped very quickly, the signs of the disease disappeared in a day. But we continued to use drops for another 3 days. I can't say anything bad about Tobradex drops, I have only positive impressions from them. Only it is better to use the drug with the permission of the doctor, there is already a very impressive list of contraindications and side effects of the drug. Be healthy!

I have never had problems with the organ of vision, but just recently woke up in the morning with red and puffy eyes. I was very scared and immediately made an appointment with an ophthalmologist. The doctor diagnosed keratoconjunctivitis. Prescribed Tobradex eye drops and recommended to frequently wash eyes with chamomile solution. The course of treatment lasted a week. I would like to note that I noticed the first results after the first application. The pus ceased to stand out, the redness began to subside. Gradually the condition stabilized. For me, the prescribed drug proved to be very effective and safe. During therapy, no unpleasant sensations arose. Recommend!
