Release form and composition

As part of a colorless transparent solution, the active ingredients are timolol maleate - 6.84 mg, tafluprost - 0.015 mg. Auxiliary components: polysorbate 80, disodium hydrogen phosphate and edetate - 4.9 and 0.5 mg, respectively, sodium hydroxide - 0.04-0.06 mg, glycerol - 22.5 mg, water for injection. (The manufacturer may substitute hydrochloric acid for sodium hydroxide).
The release form is convenient - foil bags with small dropper bottles. Each package contains 30 tubes of 0.3 ml, designed for 1 application, packed in 3 currencies, 10 pieces each. This packaging ensures absolute sterility. The currency is enclosed in a cardboard box.

The action of Tapticom

Active compounds enhance each other's action, which is why intraocular pressure decreases faster. Tafluprost, a biologically active metabolite, enhances the uveoscleral outflow of intracellular fluid, and timolol maleate, a non-selective beta-blocker, stops its synthesis.
Tapticom is absorbed through the cornea, the maximum value in the blood plasma is reached after 15-37.5 minutes, depending on the physiological status of the patient. It is excreted through secretory organs:
    tafluprost - 27-38% by the kidneys, 44-58% - through the gastrointestinal tract;
    timolol - 20% - through the urinary system, the rest with liver metabolites.
Half-life data are obtained on the basis of oral administration, T 1/2 - 4 hours.
In monotherapy, drugs based on tafluprost are administered 1 time per day, timolol - 2 times.

Indications

Eye drops Tapticom are prescribed for open-angle glaucoma, ophthalmic hypertension, introduced into a therapeutic course for the treatment of ophthalmic diseases occurring against a background of increased intraocular pressure. It is used to treat patients with individual intolerance to drugs with preservatives. It has been experimentally proven that the sterile solution is highly effective, the therapeutic effect is achieved 10-14 days after the first instillation.

Contraindications

Instillations are contraindicated in chronic obstruction of the lungs or bronchi, if there is a history of bronchial asthma or diseases that cause respiratory distress.
Contraindications for use:
    pathologies of the cardiovascular system - sick sinus syndrome, sinus bradycardia, blockade II-III degree, cardiogenic shock, heart failure;
    acute irritation of the respiratory tract of various etiologies caused by an allergic reaction or diseases from the ARVI or ARI group;
    age up to 18 years.
Despite the absence of preservatives, it is possible to develop individual intolerance, both to active ingredients and to auxiliary agents. Prescribed with caution in renal and hepatic failure.

Application instructions

Wash hands before instillation. The foil package is heated in the hand to body temperature, the lid is removed.
The head is slightly tilted back, the conjunctival sac is pulled back with the fingers and 1 drop of the solution is injected. The inner corners of the eye are pressed with fingers, without removing them for up to 2 minutes - during this time the solution is evenly distributed over the cornea. If some of the medicine enters the nasopharynx through the lacrimal canal, the concentration will decrease and the effect of the medicine will weaken. The frequency of administration is 1 time per day in each eye.
If the time of instillation is missed, then the application is continued the next day at the same time. If necessary, use several ophthalmic drugs, maintain the interval between administration - 5 - 10 minutes. No dosage adjustment is required for patients over 60 years of age.

Side effects and overdose

Most often, undesirable manifestations occur directly at the injection site. These include:
    local irritation - itching and pain in the eyes;
    eyelash discoloration and thickening;
    foreign body sensation;
    photophobia;
    the appearance of a veil before the eyes.
Uncommon: punctate keratitis, headache, erythema of the eyelids, inflammation of the anterior chamber of the eye, fatigue, blepharitis. Also, patients complained of lacrimation, conjunctival edema, discoloration of the iris, inflammation of the eyeball, expansion of capillaries, which caused redness of the eyeball, drooping of the eyelid. With prolonged use, it is possible to exacerbate bronchial asthma, shortness of breath, nervousness, insomnia, dizziness, nausea, dyspepsia.
More severe allergic manifestations are possible. Metabolic disorders, which causes hypoglycemia, decreased libido, sexual dysfunction; changes in the conduction of nerve impulses, depression, insomnia, memory loss.
There are also other side effects - from the side:
    nervous system - paresthesias, disturbances of consciousness, ischemic diseases, stroke;
    organs of hearing - ringing in the ears, impaired coordination;
    heart and respiratory organs - chest pain, arrhythmias, the development of blockade, shortness of breath. hypotension, Raynaud's syndrome, impaired peripheral blood supply.
As side effects were considered: changes in taste sensations, pain in the epigastric region, dermatological manifestations - rashes resembling psoriasis, alopecia, muscle pain, systemic lupus erythematosus, joint pain. Patients complained of general weakness, increased thirst. Ophthalmologists diagnosed corneal calcification even with minor damage to this part of the eye.
With an accidental increase in the dose during instillation, the same side effects occurred as with prolonged systemic use. In case of accidental oral use, symptomatic therapy is necessary. Excretion of active components using hemodialysis is slow.

Special instructions for Tapticom

The development of allergic reactions occurs systemically. Absorption into the bloodstream can be slowed down if, after instillation, you squeeze the nasolacrimal canal with your fingers. When treating patients with diabetes mellitus, Tapticom is used with caution, since side effects mask the symptoms of acute hypoglycemia.
The solution may cause darkening of the eyelid skin. To prevent the development of a cosmetic defect, it is enough to thoroughly wipe the eyelids after instillation, removing excess fluid from the skin of the eyelids.

Pregnancy and lactation

Women of reproductive age during treatment with Tapticom are recommended to carefully protect themselves (it is possible to use OC), since animal tests have shown a teratogenic effect on the fetus. During pregnancy, drops are prescribed, but only when the benefits to the mother's body outweigh the risk to the fetus.
Since timolol is excreted during lactation in breast milk, breastfeeding, if necessary, use ophthalmic drops is stopped. A possible side effect is the appearance of signs of blockade of β-adrenergic receptors in infants.

Age under 18

There are no clinical data on the effect of Tapticom on children, so the drug is currently not used in pediatric practice. The development of cardiac shock in children under 18 years of age was noted, therefore analogs are used with an increase in intraocular pressure.

Drug interactions

A special study of the joint use of Tapticom with other pharmacological agents was not carried out. But, based on clinical effects, it can be assumed that with simultaneous use:
    with β-blockers, cardiac glycosides, blockers of slow calcium channels and antiarrhythmic drugs, blood pressure decreases;
    with inhibitors of the isoenzyme CYP2D6 - increased heart rate.
Introduction into a therapeutic course with ophthalmic drops containing adrenaline causes pupil dilation.
In the same case, an increase in side effects from the cardiovascular system can be expected - an increase in heart rate.

Storage period
