Substances that make up drops

Combigan - eye drops, which are released in the form of a sterile solution without a pronounced odor. The liquid has a greenish yellow tint. The medication contains two active substances:
    timolol - non-cardioselective beta-blocker;
    brimonidine tartrate is an alpha2-adrenergic receptor stimulant.
Sodium hydrogen phosphate, caustic soda, sodium dihydrogen phosphate, non-alkonium chloride, purified water hydrochloric acid are additional components. They allow you to increase or decrease the pharmacodynamics and pharmacokinetics of drops, increase the shelf life, and also significantly improve many other important indicators.
The drug is packaged in convenient dropper bottles made of polypropylene, the volume of which is 10 ml. Each bottle contains 5 ml of solution. One branded pack contains 1 or 3 bottles. Depending on the duration of therapy, you can choose the required amount of medication.
The combigan is produced only in Europe in Ireland. Manufacturing company: Allergan Pharmaceuticals Ireland.

Therapeutic properties

Combigan is an antiglaucoma agent, the medicinal qualities of which are based on two active ingredients:
    Timolol is a beta-adrenergic receptor blocker. It is a group of pharmacological drugs, when introduced into the human body, beta-adrenergic receptors are blocked. The substance has no intrinsic sympathomimetic and membrane stabilizing activity.
    Brimonidine is an alpha-adrenergic receptor agonist. It is a class of sympathomimetic agents that selectively stimulate alpha-adrenergic receptors. Its selectivity is expressed in the absence of vasoconstriction (vasoconstriction) and mydriasis (pupil dilation).
Both components of the Combigan drops reduce intraocular pressure (IOP). Their combination in the composition provides a more pronounced and lasting therapeutic effect. The essence of the drug is to reduce the production of intraocular fluid and improve its outflow from the organ of vision.
More than 70% of the constituents enter the systemic circulation. Both components are excreted from the body mainly by the liver, the remainder by the kidneys. The half-life of brimonidine (T1 / 2) after topical application averages about 3 hours, timolol - about 7 hours.

Appointments and restrictions on use

The use of Combigan eye drops is indicated only in two cases:
    Open-angle glaucoma is a chronic eye pathology with a tendency to progress, characterized by increased intraocular pressure and damage to the optic nerve.
    Intraocular hypertension is a condition of increased intraocular pressure due to many pathological processes in and outside the eye.
Both of these conditions require an immediate decrease in IOP and its constant monitoring. The drug has such properties. It is often chosen by doctors for the treatment of glaucoma.
The drug is safe and well tolerated by most patients if used strictly according to the instructions for use. The list of contraindications is very wide, since each active ingredient has its own limitations for use. Drops are not prescribed for the following conditions:
    children under the age of 18;
    allergy to the components of the medication;
    cardiogenic shock;
    bronchial asthma;
    lung disease in the acute stage;
    heart failure;
    atrioventricular block;
    the period of pregnancy and breastfeeding.
Renal and hepatic failure, diabetes mellitus, severe depression, chronic heart disease, cerebral or coronary insufficiency, Raynaud's syndrome, orthostatic hypotension, thromboangiitis obliterans require careful administration.

Method of use (method, dosage)

Combigan Medicated Solution is for topical use only. The drug is shown to be used only 2 times per knocking with an interval of 12 hours, as indicated in the instructions. Drops are injected into the conjunctivitis sac of the sore eye only. Rules for instilling eye drops:
    wash your hands with soap and water;
    tilt your head back slightly, fix your gaze upward;
    pull back the lower eyelid;
    instillation is carried out into the outer corner of the palpebral fissure;
    blot excess medication with a cotton pad.
Care must be taken to prevent drops from falling on the cornea, the most sensitive part of the eye. To reduce the absorption of the drug components into the systemic circulation, after instillation, the nasolacrimal canal is pressed with fingertips for a few seconds.

Possible consequences

Only improper use of Combigan drops can provoke the development of side effects. Failure to comply with the dosage regimen may result in the following complications:
    discomfort in the organ of vision (itching, burning, foreign body sensation);
    increased lacrimation;
    photophobia;
    conjunctivitis of an allergic nature;
    dry eye syndrome;
    chronic inflammation of the edges of the eyelids - blepharitis;
    superficial damage to the epithelial layer of the cornea;
    inflammation of the cornea, in which most of the epithelium is lost - superficial keratitis.
Systemic side effects develop with prolonged therapy.
Most often, the patient develops a headache, dizziness, skin rashes, dry mouth, change in taste, runny nose, increased blood pressure, heart palpitations.

Overdose

Overdose occurs when the dosage is systematically violated, side effects are ignored. It is due to active substances that have a toxic effect on the body. In case of an overdose of drugs from the alpha2-adrenergic agonist group, there may be:
    drowsiness;
    vomiting;
    dyspnea;
    apnea;
    oppressed breathing;
    a sharp decrease in blood pressure;
    convulsions;
    bradycardia;
    decrease in body temperature;
    loss of consciousness;
    coma (rare).
In some cases, urgent hospitalization of the patient and resuscitation are required. There is a risk of death, therefore, specialist assistance is required.

Interaction

The main components of the drug, brimonidine and timolol, are able to penetrate the systemic circulation and inhibit the work of the central nervous system. For this reason, Combigan is strictly forbidden to be used simultaneously with sedatives, barbiturates, opium derivatives and alcoholic beverages. Do not combine drops with the following medicines:
    Epinephrine
    Quinidine;
    Chlorpromazine;
    Reserpine;
    Cimetidine;
    Methylphenidate.
A contraindication is also the use of beta-blockers and MAO inhibitors.

Special regulations

During instillation, do not touch the dropper tip of the mucous membrane of the organ of vision and other surfaces. Otherwise, there is a possibility of infection with the sterile ophthalmic solution.
With the development of undesirable consequences and allergic manifestations, treatment with drops is stopped. In persons on hemodialysis, during therapy, a sharp decrease in blood pressure is possible, which should be taken into account. Elderly patients do not require dosage adjustments.
Benzalkonium chloride, which is part of the drops, is an aggressive component that can damage the structure of contact lenses. Therefore, the ophthalmic device is removed prior to instillation. You can put it on after 20 minutes.
After instillation, some patients may experience temporary blurred vision, which adversely affects visual perception. Therefore, you should not get behind the wheel of a car and engage in potentially dangerous work immediately after the procedure. It is necessary to wait until vision is fully restored.

Storage conditions

The shelf life of a closed bottle is 1 year and 9 months. After opening, the solution is suitable for use within 28 days. Store the drug out of the reach of children and in a dark place where the sun's rays do not fall. The optimum storage temperature is no higher than 25 ° C. Freezing the product is strictly prohibited.

Consumer reviews

Elena Petrovna, 56 years old
Combigan was appointed by an ophthalmologist after a thorough diagnosis. The diagnosis is intraocular hypertension. He said that if no action is taken now, then glaucoma will appear. Registered for a month. I dripped 2 times a day. After the course of treatment, she underwent a second examination, the pressure stabilized. Recommended - taking vitamins for the eyes and monitoring the condition 2 times a year. The price of drops, of course, is not small, but they helped, which is much more important!
Evgeniy, 50 years old
One of the best combination drugs for today, greatly facilitating the life of not only the patient, but also the doctor. Thanks to the combination, IOP is reduced significantly in a short time. I say this because I have been living with increased pressure in my eyes for several years. I take a course with Combigan 2 times a year. During all this time, no side effects have arisen. I do not want to take any other means, the drops are completely satisfied. I recommend!
Tatiana Borisovna, 60 years old
I use Combigan drops regularly as I suffer from glaucoma. I have contraindications for the operation, so for now I am saving only with them. I mean, they keep my eyes constant. That is, the doctor did not reveal any deterioration this year. The drug worked very well for me. I was very afraid to drip them, as I have unstable blood pressure and heart problems. But against the background of treatment, no consequences arose. I hope Combigan will continue to be of great help to me. My reviews are only positive, I advise you not to be afraid and to be treated!
