Drug form, composition

The therapeutic effect of Cosopt drops is due to the two active substances that make up the composition. These include:
    dorzolamide hydrochloride;
    timolol maleate.
Auxiliary components are represented by the following components:
    benzalkonium chloride;
    sodium citrate (preservative E331);
    polyethylene glycol (PEG);
    sodium hydroxide (caustic soda);
    mannitol;
    purified water.
The medication is released in the form of a slightly viscous solution, colorless and odorless. Drops are produced in 5 ml plastic bottles of the Okumeter Plus type. The branded cardboard box contains one bottle of medicine and an annotation with all the characteristics of the drug.

Mechanism of action

Cosopt ophthalmic drops belong to the group of antiglaucoma combined medicines for local use. The ability of a medicinal solution to reduce intraocular pressure is based on the action of two active ingredients:
    Timolol is a non-selective beta-blocker with intrinsic sympathomimetic and membrane-stabilizing activity. When applied topically, timolol reduces both increased intraocular pressure and normal pressure. A decrease in IOP indicators is achieved by inhibiting the formation of eye fluid and increasing its excretion.
    Dorzolamide is a carbonic anhydrase inhibitor. This antiglaucoma agent helps to reduce the formation of bicarbonate ions with the subsequent slowing down of sodium and fluid transport. As a result, the secretion of intraocular fluid decreases.
The antihypertensive effect of drops occurs 20 minutes after instillation, reaching a peak of activity after 120 minutes. The therapeutic effect lasts for a day. After instillation, both active components enter the systemic circulation. Excreted mainly by the kidneys along with urine.

Indications, contraindications

The medication is intended to reduce IOP. Pseudoexfoliative glaucoma and open-angle glaucoma are the main indications for the appointment of Cosopt eye drops.
The drug has a fairly wide list of absolute contraindications. The drug should not be used if the patient has the following pathologies and conditions:
    susceptibility to at least one component of the drug;
    atrioventricular heart block 2-3 degrees of severity;
    chronic obstructive pulmonary disease in the acute phase;
    decreased heart rate (sinus bradycardia);
    deterioration of myocardial contractility;
    bronchial hyperreactivity syndrome;
    acute heart and kidney failure;
    extreme degree of left ventricular failure (cardiogenic shock);
    bronchial asthma;
    diseases accompanied by corneal dystrophy;
    period of bearing a child;
    lactation period;
    children under 18 years of age.
Diabetes mellitus, corneal changes, thyrotoxicosis, the process of cardiac conduction disorders of the 1st degree, circulatory disorders, urolithiasis, old age are relative contraindications, in which eye drops are prescribed carefully. Instruction (method, dosage)

As a rule, ophthalmologists recommend the standard dosage regimen indicated in the instructions. Adults and children over 18 years of age are prescribed 1 drop in each conjunctivitis sac 2 times a day.
