Instruction for Ketanov injections

You can buy Ketanov injections here

The form

To date, on the shelves of pharmacies you can find only two forms of release of Ketanov: ampoules and tablets. In suppositories, ointments and gel, this tool is not available, but the active substance can be found ketoprofen for external application under the trade name Ketorol. Tablets are packed in blisters of 10 pieces, in a box there are 20 or 100 tablets. The vials are packaged in cardboard packaging of 5 or 10 pieces.

Composition

The main active ingredient of the drug is ketorolac. Each tablet contains 10 mg of this substance, and in every milliliter of the solution for intramuscular administration - 30 milligrams.

Pharmacodynamics

Studies have shown that until the end of the mechanism of action of Ketorolaka could not figure out. The following is known:
        its introduction suppresses prostaglandin synthesis, which contributes to anesthesia;
        the most active is the S-form;
        the main component does not show sedative properties and cannot remove the feeling of fear and anxiety.
The effect of small and large doses of Ketanov injections is the duration of the analgesic effect. Clinical studies have shown that an analgesic dose of a drug can also reduce inflammation.

Pharmacokinetics

The main active ingredient of the drug is a mixture of two active forms. After use, the drug passes a fast and complete absorption.
Already almost an hour after using 30 milligrams of Ketanov injections in the patient's blood, the maximum concentration of the main substance is determined.

Linear

After intramuscular or oral use of the drug, it was found that there is no change in clearance of the racemate. This indicates a linear pharmacokinetics medication. During the dose increase, the racemate concentration increases, indicating proportional dependence.
Ketanov injections is determined in breast milk and amniotic fluid only in small quantities. Almost all ketorolac binds to blood proteins and shows a wide range of concentration values.

Metabolism

The metabolism of ketorolac occurs in the liver with the formation of hydroxylated and conjugated forms.

Removal

The main route of excretion of metabolites and unchanged substance remains is the kidneys. Only 6% of Ketorolac is excreted through the intestines.

Indications

Instructions for use of Ketanov injections indicate that this remedy is recommended for the elimination of pain syndrome of different intensity, which occurs after surgery and bothers the patient in a short period.

Dosage

Ketanov injections can be used for parenteral administration only in the hospital. Within half an hour after the injection, the patients notice a significant improvement in health, and after 120 minutes the pain disappears completely. The duration of the analgesic effect lasts up to 6 hours.
Ketanov injections can be used in ampoules for no more than two days, as the risk of negative reactions increases. To prevent this, it is necessary to switch to oral medication as soon as possible. This drug is prohibited for epidural or intraspinal administration.

For adults

The initial dose for adults is 10 mg, and if necessary, you can re-enter the drug, but not more than four times a day. After the surgery, injections Ketanov are used every few hours, but the maximum daily dose should not be more than 90 milligrams for young patients and 60 milligrams for the elderly. Opioid analgesics can be administered with ketorolac.

For the elderly

People who have reached the age of 65 need to prescribe Ketanov injections in the lowest possible therapeutic doses and very quickly switch to oral medication.

Contraindications

It is forbidden to use the drug in the presence of:
    hypersensitivity to the main or additional components of injections Ketanov;
    stomach ulcers;
    gastric bleeding;
    bronchial asthma;
    rhinitis;
    angioedema;
    disorders of the heart;
    bronchospasm;
    while taking drugs group anticoagulants;
    disorders of the liver and kidneys;
    cerebrovascular hemorrhage, which is already or is suspected;
    joint reception with aspirin, warfarin and other NSAIDs;
    hypovolemia, dehydration;
    during labor contractions during labor.
Do not administer the drug during the period of surgery on the coronary arteries.

Side effects

During use or administration of the drug, patients reported the appearance of such unpleasant sensations.
The digestive system (nausea, vomiting, stomach ache, there is a spasm, bloating, taste changes, diarrhea or bleeding occurs, and dry mouth mucosa). Also develops ulcerative stomatitis, acute pancreatitis, melena, esophagitis, Crohn's disease.
The liver and biliary pathways (the organ is disturbed, the doctors note the appearance of hepatitis, jaundice, hematomegaly, the activity of liver transaminases increases).
Nervous system (I really want to sleep, attentiveness is disturbed, euphoria appears, my head aches and is dizzy, the patient becomes anxious and irritable, falls asleep poorly).
Cardiovascular system (the heart rhythm is disturbed, skin becomes pale, the patient complains of pain in the sternum area, a heart attack or even a stroke is possible).
Blood formation organs (doctors diagnose patients with aplastic anemia, hemolytic anemia, purpura, leukopenia, eosinophilia, neutropenia).
Respiratory system (lungs and larynx swell, respiratory arrest occurs, bronchospasm aggravates).
The urinary system (the creatine in the urine rises, the quantitative content of potassium and sodium ions is disturbed, it hurts in the lumbar region, the kidneys start to work poorly, and the urine excretion is delayed).
Skin (patients notice a rash, skin flakes and thickens).
The system of hemostasis (develop bleeding from different organs, which are difficult to eliminate).
Reproductive system (a woman can not have children).
Immune system (allergy appears, skin itches, breathing becomes hard, breathing becomes whistling).
Sense organs (hearing is low, there is noise in the ears, vision is disturbed).

Application features

Like all drugs, injections Ketanov should be used with some caution. Consider the main groups of patients who should be as attentive as possible during the period of treatment with this drug.
Women who are planning to become moms should stop using Ketorolac and pass all the necessary tests. For those patients who have problems with fertilization, taking Ketanov injections is prohibited.

Liver pathologies

This drug should be very carefully prescribed to patients who have previously undergone liver treatment and have diagnoses related to this organ. If you experience pain in the right hypochondrium, you must immediately stop taking the drug, and report the incident to your doctor.

Pregnant and lactating

Clinical studies have shown that all drugs in the group of nonsteroidal anti-inflammatory drugs have a negative effect on the heart of the fetus, especially in the last stages of pregnancy. For this reason, it is forbidden to take in this period of ketorolac pain of varying severity.
Since the main component of the drug penetrates into breast milk, it is not recommended to use injections Ketanov during lactation. If the need for this tool is very great, then feeding the baby with her mother's milk for some time should be stopped.

In pediatrics

It is forbidden to prescribe injections Ketanov for the treatment of children who have not reached the age of 16.

Impact on the reaction

Some patients note that Ketanov injections cause the following manifestations:
    dizzy;
    drowsiness is felt;
    vision is impaired;
    headache;
    depression develops.
When any of the above side effects occur, it is worth refusing to drive a car or other mechanisms.

Drug interactions

Although Ketorolac does not affect the pharmacodynamic characteristics of other drugs, it is not used together with such drugs:
    Warfarin;
    Digoxin;
    Salicylates;
    Heparin.
Consider how Ketanov injections interacts with drugs from different pharmacological groups:
    Aspirin. In combination with aspirin, ketorolac binds to blood proteins much less, but the clearance remains the same. Clinical studies have not determined how dangerous this is for the body, but these drugs are not used simultaneously due to an increased risk of negative reactions.
    Probenecid - the clearance of ketorolac decreases, but there is an increase in the plasma level and a prolongation of the half-life.
    Cyclosporine - increases the possibility of nephrotoxic effect.
    Mifepristone. Ketanov injections has properties to slightly inactivate mifepristone. For this reason, ketorolac is not used for the first two weeks after using an emergency method of contraception.
    Preparations of the anticoagulant group - bleeding increases.
    Opioid analgesics. Ketanov injections has properties to enhance the therapeutic effects of opioid analgesics, so their dose can be significantly reduced.
    Ketanov injections in combination with cardiac glycosides reduces the rate of filtration in the glomeruli, disrupt the heart.
    Physicians should be extremely cautious when recommending ketorolac along with Methotrexate.
    ACE inhibitors - significantly increases the risk of kidney problems. Ketanov injections also affects the therapeutic activity of antihypertensive drugs in the direction of its reduction.
    With flucoxetine, alprazolam and thiotene, patients reported hallucinations.
    Medicines of the group of corticosteroids - very often there are bleeding from the gastrointestinal tract.
    Diuretic drugs. Ketanov injections reduces the natriuretic effects of diuretics, so it is important to monitor the work of the kidneys during treatment with these agents.
Ketanov injections may influence laboratory tests, namely, their results. During the administration of ketorolac, platelet aggregation is inhibited and bleeding is prolonged.

Overdose

In case of overdose with this drug, patients note the appearance of:
    inhibited state;
    sleepiness;
    headache;
    nausea and vomiting;
    gastrointestinal bleeding;
    pressure increase;
    difficulty breathing.
In some particularly difficult cases, an anaphylactic reaction has been reported.
Therapy for this condition is symptomatic and supportive. To date, no specific antidote has been identified that would be administered in emergency situations. First aid is as follows:
    vomiting is caused;
    after full gastric lavage, an absorbent and laxative are taken.
Since ketoprofen is almost completely bound to blood proteins, it is not advisable to force forced diuresis, alkaline urine, hemodialysis, or blood transfusion.

Storage

Ketanov injections can be used in injections for three years from the date of issue. After this time, the drug is recyclable.
The instructions for Ketanov injections indicate that it should be stored at 25 degrees, low humidity and far from children and direct sunlight.
