One of the popular hepatoprotective drugs is Carsil, produced by the Bulgarian pharmacological company Sopharma JSC.
Carsil is an effective, plant-based hepatoprotective drug. Its active component is silymarin (Silimari), which is secreted from the fruits of a medicinal plant - milk thistle.
Milk thistle (milk thistle) is a type of herbaceous plant belonging to the Astrovidae family. Raw materials for the pharmacological agent are obtained from the ripe fruits of milk thistle (in Latin Fructus Silybi mariani). They contain in large quantities the beneficial flavonoid compounds represented by silibinin, silidianin and silicristine. All of them have a phenylchromanone structure and have membrane-stabilizing, antioxidant and metabolic effects.
Silymarin interacts with free radicals found in the liver tissue and reduces their toxic effect.

Dosage form and composition

Available in the form of a dragee, which has a biconvex shape. The inner white contents on top are covered with a dense brownish shell. On the fracture side, it may have a tan color.
The dragee is packed in blisters with a total of 10 pieces and 8 blister plates in a cardboard package, in which the instructions for use are enclosed.
One dragee consists of milk thistle extract, calculated on the dry equivalent of silymarin - 35 mg.
As auxiliary and binding components used: lactose, natural starch, polyvinylpyrrolidone, microcellulose, magnesium and stearic acid salt, talc, glucose, sweetener, sodium bicarbonate.

Pharmacodynamics

Carsil contains silymarin derived from milk thistle seed extract. To the end, the mechanism of action has not been studied. However, it was proved that the hepatoprotective efficacy of silymarin is due to the negative interaction with toxic substances on the receptors in the membranes of hepatocytes, providing a strengthening and stabilizing effect.
Silymarin has a pronounced regulatory effect and is able to influence the metabolic processes at the cellular level. The membrane permeability self-regulates, inhibiting the lipoxygenase pathway of arachidonic acid oxidation, inhibiting the synthesis of active mediators of the inflammatory process, in particular Leukotriene B4. Easily interacts with free radicals and promotes the production of proteins. Restores phospholipids in damaged cells of the liver parenchyma, thereby activating the regeneration of liver tissue.
The positive effect of silymarin is determined by its antioxidant properties and the ability to significantly improve microcirculation in tissues. In general, this leads to the leveling of the symptoms of the disease and to normalization of liver function according to the main laboratory parameters: transaminases, gammaglobulin, bilirubin. The general state of health improves, and digestion is also normalized. Many patients with impaired digestion due to pathological liver diseases increase their appetite and gain weight.

Pharmacokinetics

Silymarin, getting into the intestine, is absorbed quite slowly. Amenable to enterohepatic circulation. It does not have a cumulative effect.
In laboratory studies of the kinetics of carbon-labeled silibinin 14, the greatest presence was observed in the liver, and in small doses in kidney segments and other organs.
The catabolism of a substance in the liver occurs as a result of conjugation. In the bile enzyme, glucuronides and sulfates are detected as metabolic products. The elimination half-life reaches six hours. It is excreted mainly with bile secretion (up to 80%) and in a very small dose by the urinary tract (up to 5%).


Indications

The drug Carsil is used in the following conditions:
    hepatotoxic lesion;
    supportive and rehabilitation therapy after diagnosed and treated hepatitis;
    chronic inflammation having a non-viral etiology;
    fatty hepatosis;
    with cirrhosis in a complex therapeutic treatment;
    with various hepatic lesions resulting from the long-term use of drugs, alcoholic beverages, intoxication as a preventive measure.

Reception instructions

Carsil dragee is taken orally, preferably before meals, without chewing and whole, washed down with a large volume of water.
In mild or moderate pathological conditions for adults and children over twelve years of age, it is recommended to use one or two tablets three times a day.
In severe pathologies, the dosage should be doubled to two or four tablets three times a day.
For preventive purposes, you should take two to three dragees throughout the day.
The duration of the course of treatment is determined by the doctor in private order separately for each patient, depending on the type of lesion and the severity of the disease. On average, the duration of a therapeutic course is about three months.
If, for any reason, the dose was missed, then it is necessary to take it immediately if possible. If the time has already come for taking the next portion, then it is taken as usual, but in no case a double dosage is taken to compensate for the pass.

Contraindications

It is not recommended to use a medication in the presence of hypersensitivity to the active substance or to other components.
You can not take pills for acute poisoning of various etiologies.
The drug is not used in prescriptions for children under the age of five.
According to the instructions for use, with special attention and caution should be approached for the appointment of a medication for patients suffering from various hormonal disorders, including endometriosis, uterine fibroids, malignant tumors with localization in the mammary gland, ovaries or prostate gland. In such cases, additional consultation with the attending doctor is necessary.

Features of use during pregnancy and breastfeeding

Due to the lack of reliable information on the body of a pregnant or lactating woman, it is not recommended to use this drug.


Overdose

To date, there is no reliable data confirming the facts of an overdose of Carsil. If accidental use of a multiply increased dosage of the drug has occurred, gastrointestinal lavage should be used and enterosorbents should be taken. Next, symptomatic treatment is used.

Side effects

Carsil is made from natural raw materials, therefore, as a rule, it is very well tolerated by the patient. In extreme cases, side effects may develop.
From the digestive side, reactions such as poor digestion, pain in the epigastric region, nausea, and upset stool occur. In some cases, dermatological reactions of an allergic nature were observed: rashes, itching, skin irritation, alopecia. There are rare cases of increased vestibular disorders that already existed in the patient's history.
All side effects resulting from the use of Carsil's dragee are temporary. They do not require the appointment of special therapy and disappear immediately after stopping the use of the drug.

Drug and alcohol interactions

With the combined use of the drug with the use of oral contraceptives, it is possible to reduce the effectiveness of the effects of the latter.
Carsil enhances the efficacy of sedative drugs alprozolam and diazepam, antifungal ketoconazole, cytostatic vinblastine and hypolipidemic lovastatin. The increased effect is due to the inhibitory effect of the drug on enzymes of the P450 family.
It is strictly forbidden to take alcohol-containing drinks during hepatoprotective therapy.

Storage conditions

Carsil should be stored in a specially designated place with optimal parameters for humidity and with a temperature regime not exceeding 25 degrees Celsius. The place for storing the medicine should be out of the reach of children.

Shelf life

The drug retains its qualities for 2 years. At the end of the shelf life, as well as in violation of storage conditions, it is not recommended to use the medicine.
