Dosage form, composition of Timolol

Timolol eye drops are a colorless, odorless solution. Packaged in 5 ml high density polyethylene tubes. The medicinal solution is produced with an active ingredient concentration of 0.25 or 0.5%. Each vial is equipped with a built-in dropper for accurate dosing of the medicine. The branded cardboard box contains one tube with drops, an insert sheet with a detailed description of the medication (indications, contraindications, method of application, dosage, etc.).
The active ingredient in Timolol eye drops is a beta-blocker - timolol maleate. Auxiliary components:
    Natrii hydrophosphas (sodium dihydrogen phosphate);
    Edetate disodium (ethylenediaminetetraacetic acid disodium salt);
    Disodium phosphate dihydrate (disodium phosphate dihydrate);
    Рurificati aqua (purified water);
    Benzalkonii chloridum (benzalkonium chloride);

Therapeutic action

Drops Timolol - antiglaucoma agent, non-selective beta-blocker. The action is aimed at reducing intraocular pressure. The exact mechanism for lowering IOP is not known. Perhaps this property is achieved by reducing the formation of aqueous humor and its partial withdrawal from the organ of vision. Unlike other similar medicines, Timolol does not impair vision (day, night), does not dilate the pupil. After instillation, the therapeutic effect is observed after 20-30 minutes and lasts 12 hours.
The components of the drug are able to quickly overcome the cornea. The solution enters the systemic circulation through the conjunctiva, nose, oropharynx, lacrimal tract. The maximum concentration in the blood is recorded 2 hours after instillation. The plasma detection threshold is maintained for 12 hours. It is excreted from the body in an altered form by the kidneys along with urine.

Indications for appointment

Timolol drops are indicated for use in the following cases:
    open-angle glaucoma - a persistent increase in intraocular pressure with a tendency to progress;
    angle-closure glaucoma - a pathology resulting from an increase in IOP due to the closure of the anterior chamber angle;
    secondary glaucoma - complications and consequences of various conditions or injuries of the organ of vision, which are accompanied by an increase in intraocular pressure;
    a sharp increase in IOP.
Another indication indicated in the instructions is the ineffectiveness of the use of therapeutic measures for congenital glaucoma.

Contraindications

According to the instructions, Timolol cannot be used in the presence of certain diseases and conditions. Direct contraindications:
    sensitivity to at least one component of the medication;
    inflammation of the nasal mucosa - a runny nose;
    eye diseases accompanied by clouding (dystrophy) of the cornea;
    atrioventricular block (2, 3 degrees) without an artificial pacemaker;
    decreased heart rate - sinus bradycardia;
    severe obstructive chronic pulmonary disease;

    severe left ventricular failure - cardiogenic shock;
    chronic inflammation of the airways.
Relative contraindications:

    thinning of the mucous membranes of the nose;
    diabetes;
    myasthenia gravis;
    pregnancy;
    lactation;
    childhood (especially the period of infancy).

Side effects

Method of application, dosage

Timolol eye drops are prescribed for medicinal purposes. The medicinal solution is used topically by instillation into the conjunctivitis sac. The therapy is started using drops with an active substance concentration of 0.25%. The standard dosage for newborns and adults is a drop in each conjunctivitis sac 1-2 times a day. If no positive dynamics is observed against the background of treatment, a concentrated solution of 0.5% is prescribed. Drop by drop 1-2 times a day - for children over 10 years old and adults.
As the condition improves, when IOP normalizes, the frequency of instillations is reduced to once a day. It is preferable to carry out the procedure in the morning. As a rule, the course of therapy is long and can take several months. The duration of treatment depends on the clinical efficacy of Timolol drops.

Overdose

Overdose is unlikely if the recommended dosage regimen is strictly followed. Overdose cases were recorded with an accidental increase in the dose of the drug. In this case, there is an increase in side effects and the addition of life-threatening signs:
    a sharp decrease in blood pressure;
    rapid pulse;
    cardiogenic shock;
    loss of consciousness;
    convulsions;
    collapse;
    heart failure.

Interaction with other medications

With the simultaneous use of Timolol with some drugs, undesirable consequences may occur. It is not recommended to combine drops with the following groups of medicines:
    local anesthetics;
    systemic beta-adrenergic receptor blockers;
    cardiac glycosides;
    calcium antagonists.
During treatment with the drug, it is forbidden to consume alcoholic beverages in order to avoid a sharp decrease in blood pressure.

The shelf life of a closed bottle is 2 years from the date of manufacture. After opening, the medicinal solution is suitable for use within 30 days.
