Dalacin is an antibiotic drug from the lincosamide group.

Release form and composition

Dalacin is available in the following dosage forms:
    Vaginal cream 2%: white (20 or 40 g in polyethylene or aluminum foil tubes, complete with 3 or 7 applicators (respectively), 1 set in a cardboard box);
Composition of 1000 mg vaginal cream:
    Active ingredient: clindamycin (in the form of phosphate) - 20 mg;
    Auxiliary components: cetyl palmitate - 32.1 mg; stearic acid - 21.4 mg; polysorbate 60-50 mg; sorbitan monostearate - 20 mg; propylene glycol - 50 mg; benzyl alcohol - 10 mg; cetostearyl alcohol - 32.1 mg; mineral oil - 64.2 mg; purified water - in sufficient quantity.

Indications for use

Dalacin in the form of vaginal cream and suppositories is prescribed for the treatment of bacterial vaginosis.

Contraindications

Dalacin is contraindicated for use in the presence of hypersensitivity to clindamycin, lincomycin or auxiliary components of the drug.
Additionally, Dalacin is contraindicated for the following diseases / conditions:
    A history of antibiotic-associated colitis;
    Age: up to 18 years old - for vaginal cream and suppositories; up to 12 years - for a gel for external use (there are no data on the effectiveness and safety of using Dalacin in this group of patients);
    History of hypersensitivity to lincomycin or clindamycin (for topical gel).

Method of administration and dosage

When treating bacterial vaginosis, the following are usually prescribed:
    Vaginal cream: 5000 mg of cream (1 full applicator, approximately 100 mg of clindamycin) should be inserted into the vagina. The procedure is best done before bedtime. Course duration - 3 or 7 days;
Dalacin in the form of a gel is used externally in the treatment of acne vulgaris. The drug is applied in a thin layer to the affected area (dry, clean skin). Frequency rate of application - 2 times a day. The recommended course duration is 6-8 weeks. According to indications, it is possible to use Dalacin for 6 months. If, after applying the gel for several months, the effectiveness of the treatment decreases, you need to take a break for 4 weeks.

Side effects

In the treatment of bacterial vaginosis with Dalacin in the form of a vaginal cream and suppositories, disorders may develop from some body systems:
    Reproductive system: vulvovaginitis, vaginal candidiasis, uterine bleeding, irritation of the mucous membrane of the vagina and vulva, vaginal infections, Trichomonas vaginitis, vaginal pain, abnormal labor, menstrual irregularities, vaginal discharge, endometriosis;
    Urinary system: urinary tract infections, dysuria, proteinuria;
    Digestive system: taste perversion, flatulence, nausea, diarrhea, vomiting, indigestion, constipation, bad breath, gastrointestinal disturbances;
    Skin: erythema, itching, rash, skin candidiasis, maculopapular rash, urticaria;
    Central nervous system: dizziness, headache, vertigo;

Special instructions

With the development of diarrhea after the use of Dalacin orally or intravaginally, the possibility of developing pseudomembranous colitis should be considered. The disease can also appear 2-3 weeks after stopping therapy. Its main symptoms include: diarrhea, leukocytosis, fever, abdominal pain (sometimes accompanied by mucus and blood with feces). If necessary, diagnostic and therapeutic measures should be taken. With external use of the drug, cases of colitis are rare, however, care must be taken and, in the event of prolonged or severe diarrhea, the use of the gel should be canceled.
For cervical infections caused by Chlamydia trachomatis, Dalacin capsule monotherapy is effective for complete cure.
Clindamycin should not be used to treat meningitis.
When prescribing Dalacin in high doses, it is necessary to control the concentration of clindamycin in plasma. With prolonged therapy, kidney and liver function tests should be carried out regularly.
Before prescribing Dalacin for the treatment of vulvovaginitis, the following possible causative agents should be excluded: Chlamydia trachomatis, Trichomonas vaginalis, Candida albicans, Neisseria gonorrhoeae and the Herpes simplex virus.
It should be borne in mind that intravaginal use of the drug can cause increased growth of microorganisms insensitive to its action, especially yeast-like fungi.
With intravaginal use of the drug, sexual intercourse should be avoided, as well as the use of other types of products administered intravaginally (for example, syringes, tampons).
The use of Dalacin in the form of a cream or suppository during menstruation is not recommended. In this case, the start of therapy should be postponed until the end of menstruation.
Dalacin contains components that affect the strength of rubber or latex products (contraceptive vaginal diaphragms, condoms). The use of such products during therapy and within 3 days after using the drug is not recommended.
It is necessary to avoid getting the gel for external use on the mucous membranes of the oral cavity and eyes. After applying the drug, wash your hands thoroughly. In case of accidental contact with sensitive surfaces (abrasions on the skin, eyes, mucous membranes), rinse them abundantly with cool water.

Drug interactions

Clindamycin should not be taken by mouth at the same time as erythromycin and chloramphenicol.
There is cross-resistance of microorganisms to lincomycin and clindamycin.
Clindamycin should be used with caution in patients receiving other peripheral muscle relaxants.
With the simultaneous use of Dalacin orally with opioid (narcotic) analgesics, which have a central inhibitory effect on respiratory function, a more pronounced respiratory depression may develop, up to apnea.
The risk of developing pseudomembranous colitis increases with the simultaneous use of Dalacin with antidiarrheal drugs that reduce gastrointestinal motility.
The spectrum of antimicrobial action of clindamycin is expanded by aminoglycosides.
The simultaneous use of vaginal cream and suppositories with other drugs for intravaginal administration is not recommended.
