Composition

100 g of gel contains the active substance: ibuprofen 5 g and auxiliary substances: hyetillosis 1.8 g, sodium hydroxide QS (0.93 - 1 g), benzyl alcohol 1 g, isopropanol 5 g, purified water 86.2 g

Description

Transparent or slightly opalescent colorless homogeneous gel with a characteristic smell of isopropyl alcohol.
Pharmacotherapeutic group: nonsteroidal anti-inflammatory drug (NSAIDs).

Pharmacological properties

Nurofen Express gel in the dosage form of the gel for external use is easy to apply and quickly absorbed into the skin.

Pharmacodynamics

Nurofen Express gel belongs to the group of nonsteroidal anti-inflammatory drugs (NSAIDs).
It has a double effect: analgesic and anti-inflammatory. Ibuprofen, being a derivative of propionic acid, non-selectively blocks cyclooxygenase 1 (COX-1) and cyclooxygenase 2 (COX-2), as a result of which it inhibits the synthesis of prostaglandins - mediators of pain, inflammation and hyperthermic reaction.
When applied to the skin, the gel has an additional cooling effect due to the rapid evaporation of the benzyl alcohol contained in the composition.

Pharmacokinetics

After application to the skin, ibuprofen is found in the epidermis and dermis after 24 hours. The maximum concentration in plasma of ibuprofen when it is applied topically is 5% of the level of maximum concentration when using oral forms of ibuprofen. Clinically significant systemic absorption practically does not occur.
Metabolized in the liver. Excreted by the kidneys (in unchanged form, not more than 1%) and, to a lesser extent, with bile.

Indications for use

As a local anesthetic and anti-inflammatory agent for conditions such as muscle pain, back pain, arthritis, pain with ligament injuries and sprains, sports injuries and neuralgia.


Contraindications

- Hypersensitivity to ibuprofen or other components that make up Nurofen Express gel.
- A history of hypersensitivity reactions (bronchial asthma, rhinitis, angioedema, urticaria) in response to the use of acetylsalicylic acid or other NSAIDs.
- Violations of the integrity of the skin at the site of application of Nurofen Express gel (including infected abrasions and wounds, weeping dermatitis, eczema).
- Children's age up to 14 years.
- Pregnancy III trimester.

Bronchial asthma or allergic diseases in the exacerbation stage or in the history - development of bronchospasm, renal failure, liver failure, peptic ulcer of the stomach, including in the anamnesis, enteritis, colitis, hemorrhagic diathesis, pregnancy I-II trimester, breastfeeding period, wind smallpox.

Use during pregnancy and during breastfeeding

Use of Nurofen Express gel in the III trimester of pregnancy is contraindicated. The use of the drug in the I-II trimesters of pregnancy should be avoided; if necessary, use of the drug should be consulted with a doctor.
There is evidence that ibuprofen in small quantities can penetrate into breast milk without any negative consequences for the health of the infant, so usually with a short-term use of the need to stop breastfeeding does not occur.
If necessary, long-term use of Nurofen Express gel should consult a doctor to decide whether to stop breastfeeding for the period of use of the drug.

Dosage and administration

For external use only.
Carefully read the instructions before using Nurofen Express gel.
Nurofen Express Gel is prescribed for adults and children over 14 years old.
When using the tube for the first time: open the lid, check that the safety foil on the neck of the tube is intact. Open the foil by pushing it with the outside of the cover.
Squeeze 4 to 10 cm of Nurofen Express gel out of the tube (equivalent to approximately 50 to 125 mg of ibuprofen) and gently rub the gel into the skin until completely absorbed.
Immediately after using the drug, you must wash your hands.
Re-use the drug should be no earlier than 4 hours. Do not use the gel more than four times within 24 hours. The maximum daily dose of 500 mg.
If within 2 weeks of using Nurofen Express gel the symptoms persist or worsen, it is necessary to stop treatment and consult a doctor.
Do not exceed the specified dose.

Side effect

The frequency of adverse reactions was estimated based on the following criteria: very frequent (≥ 1/10), frequent (from ≥ 1/100 to <1/10), infrequent (from ≥ 1/1000 to <1/100), rare ( ≥ 1/10000 to <1/1000), very rare (> 1/10000), frequency unknown (frequency cannot be determined based on available data).
The following adverse reactions were observed with short-term use of ibuprofen at a dose not exceeding 500 mg / day. In the treatment of chronic conditions and long-term use may cause other adverse reactions.
- Immune system disorders
The frequency is unknown: hypersensitivity reactions - nonspecific allergic reactions and anaphylactic reactions, reactions of the respiratory tract (bronchial asthma, including its exacerbation, bronchospasm, shortness of breath, dyspnea), skin reactions (itching, urticaria, purpura, angioedema, exfoliative and bullous dermatosis, including toxic epidermal necrolysis (Lyell's syndrome), Stevens-Johnson syndrome, erythema multiforme).
- Violations of the gastrointestinal tract
Frequency unknown: abdominal pain, dyspepsia.
- Kidney and urinary tract disorders
Frequency unknown: impaired renal function.

Overdose

The probability of an accidental overdose of the drug in the form of a gel is minimal.
In children, overdose symptoms may occur after ingestion of a dose in excess of 400 mg / kg body weight. In adults, the dose-dependent effect of overdose is less pronounced. The half-life of Nurofen Express gel in overdose is 1.5-3 hours.
If the drug was accidentally taken by mouth, the following symptoms may occur: headache, vomiting, low blood pressure.
Treatment: gastric lavage (only within an hour after administration), administration of activated carbon, alkaline drinking, forced diuresis, symptomatic therapy.

Interaction with other drugs

With caution to apply simultaneously with the following drugs:
    Anticoagulants and thrombolytic drugs,
    Antihypertensives,
    Acetylsalicylic acid,
    other NSAIDs.
It should be borne in mind that even with topical administration of ibuprofen, its systemic effect cannot be completely excluded and, theoretically, with simultaneous use of ibuprofen gel with other NSAIDs, side effects may increase.

Avoid getting the drug in the eyes, lips and other mucous membranes, damaged skin.
Do not use in combination with an occlusive (airtight) medical bandage.
Avoid excessive sunlight on the application.
The use of NSAIDs in patients with chickenpox may be associated with an increased risk of developing severe purulent complications of infectious and inflammatory skin diseases and subcutaneous fat (eg, necrotizing fasciitis). In this regard, it is recommended to avoid the use of Nurofen Express gel for chickenpox.
