Picoprep is a laxative drug.

Release form and composition

The drug is produced in the form of an effervescent powder for the preparation of a solution for oral administration, which is a crystalline mass of white with a weak orange aroma. The finished solution is opaque, white or white with a yellowish tint, with a characteristic orange taste. The powder is packaged in four-layer sachets of 16.1 g, in a cardboard box 2 bags and instructions for use Picoprep.
Composition for 1 sachet:
    active substances: sodium picosulfate monohydrate - 10 mg; magnesium oxide - 3.5 g; anhydrous citric acid - 12 g;
    auxiliary components: sodium saccharinate dihydrate, potassium bicarbonate, orange flavor [lactose, ascorbic acid, dry orange extract, butylhydroxyanisole (E320), xanthan gum (E415)].

Pharmacodynamics

The active components of the drug are sodium picosulfate and magnesium citrate, which is formed from magnesium oxide and citric acid when the powder is dissolved. Sodium picosulfate exhibits its properties in the colon, exerting a local laxative effect. Magnesium citrate holds water in the colon, acting as an osmotic laxative.
Due to the powerful leaching effect and stimulation of peristalsis, bowel cleansing occurs. Such preparation is necessary before performing surgical operations, as well as before endoscopic or radiological examinations.

Pharmacokinetics

The components of the drug show their activity in the large intestine and are practically not absorbed into the blood.

Indications for use

Picoprep is used to cleanse the intestines before performing surgical operations and in preparation for colonoscopy or x-ray examination.
The drug can not be used as a regular laxative.

Picoprep, instructions for use: method and dosage

An effervescent solution prepared from Picoprep powder is taken orally.
The day before the procedure, it is necessary to reduce the number of meals. In order to prevent dehydration during the period of use of the drug, it is recommended to drink a sufficient amount of liquid.
To prepare the solution, the contents of one sachet are poured into 150 ml of water, stirred for 2-3 minutes, cooled to a comfortable temperature and drunk.
Picoprep can be prescribed only to children and adolescents over 9 years old, and adult patients.
If the operation or examination is scheduled for the first half of the day, then the contents of the first sachet from the package must be taken after lunch or in the early evening (at 16-18 hours), drinking at least 1250 ml (5 glasses of 250 ml) of clear liquid or water (you can use plain water, coffee, tea, non-carbonated soft drinks or fruit juice without pulp). The second bag is taken in the interval from 22 to 24 hours, drinking at least 750 ml (3 cups of 250 ml) of a clear liquid or water. The last glass is recommended to drink no later than 1 hour before the planned procedure.
If the operation or examination is scheduled for the afternoon, the contents of the first sachet from the package must be taken in the evening (at 17-21 hours) on the day that precedes the procedure, drinking Picoprep at least 1250 ml (5 cups of 250 ml) of clear liquid or water . The second bag is taken on the morning of the day of the study or operation (5–9 hours before the procedure), drinking at least 750 ml (3 cups of 250 ml) of clear water or liquid. The last glass is drunk no later than 1 hour before the planned procedure.

Side effects

    digestive system: often - nausea, pain in the rectum; rarely - abdominal pain, vomiting, loose stools; in isolated cases - transient aphthoid ileum ulceration;
    metabolism: rarely - hypokalemia, hyponatremia;
    nervous system: often - headache; rarely - confusion, epilepsy, convulsions, a large seizure (without hyponatremia);
    skin and subcutaneous fat: rarely - itching, rash (including maculopapular and erythematous), purpura, urticaria;
    immune system: rarely - hypersensitivity reactions, anaphylactic reactions.

Overdose

An overdose of Picoprep can lead to profuse diarrhea and further dehydration. It is necessary to take measures to restore the water-electrolyte balance.

Special instructions

Picoprep is not allowed to be used as a laxative on a regular basis (not as preparation for surgery or research).
With signs of hyponatremia, urgent measures are needed to restore the water-electrolyte balance.
The composition of the drug includes potassium bicarbonate, which should be considered when it is prescribed to patients who adhere to a low-calorie diet.
Orange flavoring contains lactose, therefore the drug is contraindicated in people with lactase deficiency, lactose intolerance and glucose / galactose malabsorption syndrome.
With a significant decrease in renal function, an increase in the plasma concentration of magnesium is possible. In this case, it is necessary to use another laxative drug.
In case of insufficient consumption of electrolytes and water, a clinically significant deficiency of these components may develop (especially in debilitated patients).
The total duration of preparation for bowel cleansing should be no more than 24 hours, since with the lengthening of this period, the risk of water-electrolyte disturbances increases.
Other powdered laxatives decrease the effectiveness of the drug.

Influence on the ability to drive vehicles and complex mechanisms

Since the drug can cause undesirable effects on the part of the nervous system, caution must be exercised during its use when driving vehicles and doing other work requiring a high concentration of attention and a quick reaction.

Pregnancy and lactation

Special studies on the use of Picoprep during pregnancy have not been conducted, but since sodium picosulfate stimulates intestinal motility, it should not be prescribed to pregnant women in the first trimester. In the second and third trimesters, the use of the drug is possible only after assessing the expected benefits and possible risks.
Laxative components are not secreted with breast milk. Despite this, during lactation, the drug Picoprep is used with caution.

Use in childhood

Picoprep is contraindicated in children under 9 years of age.

With impaired renal function

A laxative should not be prescribed to patients with severe impaired renal function.
For mild or moderate impaired renal function, Picoprep is used with caution.

Drug interaction

Picoprep can affect the degree of absorption of systemically taken oral medicines (antibiotics, hypoglycemic and antiepileptic drugs, oral contraceptives). Cases of epileptic seizures have been reported in patients taking a laxative in the presence of epilepsy, which was previously compensated by antiepileptic drugs. In such patients, Picoprep is used with extreme caution.
Diuretics, cardiac glycosides, and corticosteroids, taken in conjunction with a laxative, increase the risk of hypokalemia.
The likelihood of disturbing the balance of electrolytes and water increases with simultaneous use with non-steroidal anti-inflammatory drugs and drugs that can cause cerebral salt-like syndrome and / or Parkhon's syndrome (syndrome of inadequate secretion of antidiuretic hormone). Such drugs include antipsychotics, selective serotonin reuptake inhibitors, carbamazepine, tricyclic antidepressants, etc.

Terms and conditions of storage

Keep out of reach of children at a temperature not exceeding 25 ° C.
Shelf life is 3 years.
The drug must be used immediately after opening. An opened but unused powder bag must be destroyed.

Reviews

According to reviews, Picoprep is a good tool for cleansing the intestines in order to prepare for colonoscopy, operations and other studies. The drug acts gently, does not cause intestinal cramps. The solution tastes good, its volume is small, which is also very convenient.
Some users complain of side effects of a laxative (severe weakness, dizziness, and nausea). Also, patients note that Picoprep is difficult to find on sale (not present in all pharmacies), and its cost is quite high.
