Lorista is a drug used for high blood pressure and heart failure.
Pharmachologic effect
Lorista's active ingredient is losartan, which has the ability to block angiotensin 2 receptors in the heart, kidneys, blood vessels, adrenal cortex, which leads to a decrease in vasoconstriction (narrowing of the lumen of the arteries), a decrease in total peripheral resistance and, as a result, to a decrease in blood pressure.
With Lorista's heart failure, reviews confirm that it increases the endurance of patients during physical exertion, and also prevents the development of myocardial hypertrophy. The maximum concentration of losartan in the blood can be observed 1 hour after oral administration of Lorista, while the metabolites formed in the liver begin to act in 2.5-4 hours.
Lorista N and Lorista ND are combined preparations, the active ingredients of which are losartan and hydrochlorothiazide. Hydrochlorothiazide has a pronounced diuretic effect, which is due to the ability of the substance to influence the processes of the second phase of urine formation, which is the reabsorption (absorption) of water, magnesium, potassium, chlorine, sodium ions, and also to delay the excretion of uric acid and calcium ions. Hydrochlorothiazide has antihypertensive properties, which are explained by its action aimed at expanding arterioles.
The diuretic effect of this substance can be observed within 1-2 hours after the application of Lorista N, while the hypotensive effect develops after 3-4 days.
Indications for use
The instruction recommends to use the drug when:
arterial hypertension;
left ventricular hypertrophy and arterial hypertension in order to reduce the risk of stroke;
chronic heart failure as part of a combination treatment;
nephrology in patients with type 2 diabetes mellitus in order to reduce protenuria (the presence of protein in the urine).
According to the instructions, Lorista N is prescribed if necessary, combined treatment with antihypertensive drugs and diuretics.
Contraindications
Lorista, the use assumes a preliminary consultation with a doctor, is not prescribed for low blood pressure, dehydration, hyperkalemia, lactose intolerance, syndrome of impaired absorption of glucose and galactose, hypersensitivity to losartan. You should refuse to use Lorista for pregnant and breastfeeding patients, as well as for persons under 18 years of age. Lorista N, in addition to the above contraindications, is not prescribed for severe renal or liver dysfunction and anuria (lack of urine in the bladder).
With caution, the tablets should be taken by persons with renal or hepatic insufficiency, with violations of the water and electrolyte balance, with a reduced volume of circulating blood.
Instructions for use
Lorista is available in the form of tablets containing 100, 50, 25 or 12.5 mg of losartan potassium. The drug should be taken orally once a day.
With arterial hypertension, to reduce the risk of stroke, as well as to protect the kidneys in patients with diabetes mellitus, the instruction recommends taking Lorista tablets in a daily dose of 50 mg. If it is necessary to achieve a more pronounced effect, the dose can be increased to 100 mg per day. As evidenced by reviews, Lorista develops its antihypertensive effect within 3-6 weeks of treatment. With the simultaneous administration of high doses of diuretics, the use of Lorista should be started with 25 mg per day. Also, a lower dose of the drug is recommended for people with impaired liver function.
In case of chronic insufficiency, the drug, the use involves the simultaneous administration of diuretics and cardiac glycosides, is used according to a certain scheme. During the first week of treatment, Lorista should take 12.5 mg per day, then every week the daily dose should be increased by 12.5 mg. If the drug is taken correctly, the fourth week of treatment will start with 50 mg of Lorista per day. Further treatment with Lorista should be continued, taking a maintenance dose corresponding to 50 mg.
Lorista N is a tablet containing 50 mg of losartan and 12.5 mg of hydrochlorothiazide.
Lorista ND tablets contain the same combination of substances, only twice as many - 100 mg of losartan and 25 mg of hydrochlorothiazide.
In case of arterial hypertension, the recommended daily dose of Lorista N is 1 tablet, if necessary, it is allowed to take 2 tablets per day. If the patient has a decrease in the volume of circulating blood, the drug should be started with a daily dose of 25 mg. Lorista N tablets should be taken after correction of the circulating blood volume and discontinuation of diuretics.
As evidenced by the reviews, it is advisable for Lorista N to take it at risk of cardiovascular diseases if monotherapy with losartan did not help to reach the target blood pressure level. The recommended dose of the drug per day is 1-2 tablets.

gout, hyperkalemia, anemia;
angioedema, skin rash, itching, urticaria.
As a rule, the listed undesirable effects of Lorista have a short-term and weak effect.
