Release form and composition

Clinico-pharmacological group: local anesthetic. Novocaine dosage forms:
    Rectal suppositories (5 pieces in a planimetric package, in a cardboard bundle 2 packs; 5 pcs in a contour boxless package, in a cardboard bundle 2 packs).
    Solution for injection 0.5%: colorless, clear liquid (2, 5 or 10 ml in an ampoule; in a cardboard bundle 10 ampoules complete with scarifier or ampule knife).
    Solution for injection 0.25% (1, 2, 5 or 10 ml in an ampoule, in a cardboard bundle 10 ampoules).
    Solution for injection 1% (1, 2 or 5 ml in an ampoule, in a cardboard bundle 10 ampoules).
    Solution for injection 2%: colorless, transparent or slightly colored liquid (2 or 5 ml in an ampoule, in a cardboard bundle 10 ampoules complete with a scarifier or ampule knife; 5 ampoules in a blister pack, in a cardboard bundle 2 packs complete with scarifier or ampoule knife. In packs with ampoules with a ring or a break point do not put a scarifier or ampoule knife).
Active ingredient: procaine hydrochloride (in 1 ml of the solution - 2.5; 5; 10 or 20 mg; in 1 suppository - 100 mg).

What is Novocaine used for?

Injection solution is shown in such situations:
    Local anesthesia (infiltration, conduction, epidural) during various surgical interventions;
    Novocaine blockade (perirephalic and vagosympathetic);
    Circulatory and paravertebral blockade with sciatica, neurodermatitis, eczema and other painful conditions;
    Spasms of peripheral vessels (for stopping);
    Frostbite (to improve microcirculation in the pre-reactive period).


pharmachologic effect

The drug has a moderate anesthetic effect, reduces the excitability of the motor areas of the brain, myocardium, eliminates smooth muscle spasms.
    When administered intravenously, the drug reduces pressure, has an antiarrhythmic effect, removes from shock. In large doses, the use of Novocaine can disrupt muscle conductivity and cause seizures.
Intramuscular administration of Novocaine solution is effective for elderly patients with diseases associated with disorders of the nervous system.

Instructions for use

Depending on the disease, the dosage regimen and method of administration of the drug differ. Depending on the indications, Procaine solution is administered intramuscularly, orally, intravenously, rectally, by electrophoresis and intraosseous.
Dosage for procaine solution only 5 mg / ml (0.5%):
    For infiltration anesthesia, 350-600 mg (70-120 ml) are administered. Higher doses for infiltration anesthesia for adults: the first single dose at the beginning of the operation is no higher than 0.75 g (150 ml), then for each hour of the operation no more than 2 g (400 ml) of the solution.
    In case of perirenal blockade (according to AV Vishnevsky), 50-80 ml are injected into the renal cellulose.
    With a circular and paravertebral block, 5-10 ml are injected intradermally. When vagosympathetic blockade enter 30-40 ml.
    To reduce the absorption and lengthen the action under local anesthesia, a 0.1% solution of epinephrine hydrochloride is additionally injected, 1 drop per 2-5-10 ml of procaine solution.
The maximum dose for use in children over 12 years old is 15 mg / kg.

Contraindications

According to the instructions Novocaine contraindicated:
    fibrous changes in the tissues (during anesthesia using the method of creeping infiltration);
    hypotension, bleeding, septicemia, shock (when conducting subarachnoid anesthesia);
    hypersensitivity to procaine.
Novocaine cautiously prescribed for:
    acute blood loss;
    cardiovascular failure;
    liver failure;
    inflammatory diseases;
    conditions involving reduced blood supply to the liver;
    infection of the injection sites.
Careful medical control requires the use of Novocaine for children (up to 18 years), elderly patients (over 65), as well as seriously ill and debilitated patients, women during childbirth.

Side effects

After taking Novocaine may occur:
    headache;
    seizures;
    sleepiness;
    dizziness;
    loss of consciousness;
    weakness;
    nystagmus;
    tremor;
    paresthesia;
    pain in the sternum;
    respiratory paralysis;
    bradycardia;
    leg paralysis;
    collapse;
    pressure surges;
    arrhythmias;
    involuntary feces;
    skin rashes;
    vomiting;
    hypothermia;

Overdose symptoms: pallor of the skin and mucous membranes, dizziness, nausea, vomiting, "cold" sweat, paresthesias around the mouth, numbness of the tongue, increased breathing, tachycardia, low blood pressure, until collapse, apnea, methemoglobinemia.
The effect on the central nervous system is manifested by a sense of fear, hallucinations, convulsions, motor agitation.
