Pharmachologic effect of Magnesium sulfate

Magnesium sulfate is a drug whose therapeutic effect depends on the method of application.
So, when ingestion of magnesium sulfate has a laxative and choleretic effect.
The laxative effect of the drug is due to its poor absorption, which leads to the creation of high osmotic pressure in the intestine, which entails the accumulation of water, dilution of intestinal contents and, as a result, increased peristalsis.
The choleretic effect of magnesium sulfate is explained by its reflex effect on the mucous membrane of the duodenum.
This tool is an antidote for poisoning with salts of heavy metals.
Magnesium sulfate begins to act after 0.5-3 hours after ingestion, the therapeutic effect persists for 4-6 hours.
Intravenous and intramuscular administration of magnesium sulfate has a diuretic, vasodilator, antiarrhythmic, anticonvulsant, antispasmodic and sedative effect. Parenteral administration of large doses of the drug can cause tocolytic (prevents premature birth), sleeping pills, curariform (inhibits neuromuscular transmission) and the narcotic effect of the drug.
The use of magnesium sulfate can reduce the excitability of the respiratory center, reduce blood pressure, which is due to the soothing properties of the drug, and increase urination.
After intravenous administration, the drug begins to act almost instantly, maintaining its therapeutic effect for half an hour. After intramuscular administration of magnesium sulfate begins to act after 60 minutes and continues for the next 3-4 hours.

Indications for use of magnesium sulfate

Magnesium sulfate powder, which is intended for the preparation of a suspension or solution for oral administration, is prescribed for: constipation, cholecystitis (acute inflammatory process in the gallbladder), cholangitis (inflammation of the bile ducts), gallbladder dyskinesia (disturbances in reduction), duodenal sounding, cleansing intestines before diagnostic procedures.
Magnesium sulfate in ampoules containing a solution for intramuscular and intravenous administration is used for:
    hypomagnesemia;
    cramps with gestosis (a disorder of the body's systems during pregnancy);
    the threat of premature birth;
    arterial hypertension;
    epileptic syndrome;
    encephalopathies (non-inflammatory brain diseases);
    eclampsia (critically high blood pressure in pregnant women or women in labor);
    urinary retention.
Magnesium sulfate in any dosage form is effective for poisoning with salts of heavy metals such as arsenic, tetraethyl lead, and mercury.

Contraindications

The instruction does not recommend oral administration of magnesium sulfate for rectal bleeding, appendicitis, intestinal obstruction, dehydration, hypermagnesemia.
Magnesium sulfate in ampoules for parenteral use is contraindicated in case of depression of the respiratory center, arterial hypotension, severe bradycardia, AV block. Do not use the drug 2 hours before birth.
Magnesium sulfate in any dosage form is not prescribed for chronic renal failure and hypersensitivity to the drug.

Instructions for use Magnesium sulfate

Before ingestion, magnesium sulfate powder is dissolved in 100 ml of slightly heated water. To obtain a laxative effect, the contents of one packet (20-30 g) are used as a single dose for adults, and for children, the amount of powder in grams should correspond to the child's age in years.
Magnesium sulfate 20-25% solution is taken orally if necessary to obtain a choleretic effect - 3 times a day for 15 ml. Duodenal sounding involves the introduction of a probe of 10% (100 ml) or 25% (50 ml) of a warm solution. To cleanse the stomach of salts of heavy metals, either washing with a 1% solution is carried out, or the patient takes 20-25 g of magnesium sulfate powder, dissolved in 200 ml of water, inside. The daily dose of the drug for adults should not exceed 40 g of the drug.
Magnesium sulfate in ampoules is intended for parenteral administration, which should be carried out slowly - no more than 3 ml should be administered during the first three minutes. Typically, the drug is used 1-2 times a day, 5-20 ml, using a 20-25% solution. The duration of treatment is 2-3 weeks.
To cleanse the body of salts of heavy metals of magnesium sulfate, the instruction recommends that 5-10 ml be administered intravenously in the form of a 5-10% solution.
Relieving seizures in children involves the intramuscular use of the drug, at the rate of 20-40 Magnesium per kg.

Side effects

Magnesium sulfate can provoke such unwanted reactions of the body, such as:
    exacerbation of diseases of the digestive tract, diarrhea, vomiting, nausea, flatulence, thirst, in case of renal failure - signs of hypermagnesemia (dizziness);
    violation of electrolyte balance, which manifests itself in the form of seizures, arrhythmias, confusion, asthenia, and increased fatigue.
Initially, hypermangiemia can be expressed as a decrease in blood pressure, headache, shortness of breath, weakness, vomiting, nausea, blurred speech, bradycardia, sudden “flushing” of blood to the face.
An excessive concentration of magnesium in the blood serum has the following symptoms: depression of the respiratory center, loss of deep tendon reflexes, impaired conduction of the heart, cardiac arrest, anxiety, increased sweating, increased urine formation, deep sedation (depression of consciousness), uterine atony (a sharp decrease in tone and loss muscle contractility).

Additional Information

The instruction recommends storing magnesium sulfate at room temperature, the fulfillment of this condition will allow the drug to remain suitable for 5 years.

Magnesium deficiency

Magnesium is involved in many processes occurring in the body. Therefore, even a person who regularly consumes micronutrient-rich foods can have a deficiency.
Symptoms of a lack of magnesium in the body
    fatigue, irritability;
    involuntary muscle twitching;
    cramps
    numbness;
    heart rhythm disturbance;
    insomnia.
Of course, in themselves, these signs without a blood test can mean that a person has worked hard, tired, and just needs a rest. Here, to find out the cause of ill health, and you need to visit a doctor.

Causes of Magnesium Lack

    malnutrition;
    constant stress;
    taking diuretics and certain antibiotics;
    drug and alcohol use;
    nicotine and caffeine;
    unbalanced mono-diets;
    significant physical exertion;
    passion for fast food and refined foods;
    frequent diarrhea without compensation with a special drink and diet;
    excess fatty foods;
    exorbitant passion for sweets;
    the use of diuretics, laxatives and hormones without the supervision of a doctor;
    deficiency of vitamins of group B.

Causes of magnesium deficiency in women

    taking hormonal contraceptives;
    pregnancy and lactation, when the need for a trace element increases.
At risk are:
    the elderly - with age, magnesium is absorbed worse from food, it is necessary to take medications or dietary supplements;
    children - the growing body needs high doses of the trace element, even with proper nutrition, it is excreted during active games and emotional outbursts;
    patients with diabetes, obesity;
    people suffering from chronic diseases of the endocrine system, gastrointestinal tract;
    symptoms of magnesium deficiency in the body are often manifested in men and women with an unstable nervous system.

Forms of magnesium and their digestibility

The Magnesium content in the drug can tell little to a person far from pharmacy. A medicine can have a lot of active substance, but if it is in a poorly accessible form, only a small part will be absorbed, the rest will be eliminated from the body. Magnesium oxide belongs to such preparations. The trace element contained in sulfuric acid salts is also poorly absorbed.
In its pure form, Magnesium generally has a low degree of absorption — not more than 45%. But the more the body needs magnesium, the better it is absorbed. Regardless of form. If you need magnesium, which is better absorbed, you should choose between citrate and chelate. Some sources advise adding lactate and glycinate to the list.
