Domstal is an antiemetic drug, a central blocker of dopamine receptors.

Release form and composition

Dosage form - tablets: flat, round, almost white or white, with a chamfer and a risk on one side (10 pcs. In a blister of aluminum foil and PVC film, in a pack of cardboard 1 or 3 blisters).
The composition of 1 tablet includes:
    active substance: domperidone - 10 mg;
    excipients: lactose - 114 g, starch - 38 mg, potassium hydrogen phosphate - 29.5 mg, gelatin - 3.5 mg, talc - 3 mg, magnesium stearate - 2 mg.

Pharmacodynamics

Domperidone is a dopamine antagonist and, like metoclopramide and few antipsychotics, has antiemetic properties. But in comparison with them, domperidone does not pass well through the blood-brain barrier. Its use rarely causes extrapyramidal side effects, especially in adult patients, but the use of domperidone stimulates the release of prolactin by the pituitary gland. The antiemetic effect of Domstal is possibly due to a combination of gastrokinetic (peripheral) action with antagonism to dopamine receptors in the trigger zone of the brain.
The use of domperidone inward increases the duration of duodenal and antral contractions, speeds up gastric emptying - the output of solid fractions in patients (in whom this process was slowed down), liquid and semi-solid fractions in healthy people, and also increases the pressure of the sphincter of the lower esophagus in healthy people. Domperidone has no effect on gastric secretion.

Pharmacokinetics

The bioavailability of Domstal after oral administration (by mouth) reaches 13-17% (the effect of the first passage through the liver). The half-life is 7.5 hours, with renal failure it can increase to 20.8 hours. Domperidone is subject to extensive and rapid metabolism. It is excreted through the intestines - 66%, by the kidneys - 33%, including unchanged - 10 and 1%, respectively.

Indications for use

    a combination of dyspeptic symptoms, often associated with delayed emptying of the stomach, gastroesophageal reflux, esophagitis (a feeling of fullness in the epigastrium, pain in the upper part and a feeling of bloating; flatulence, belching; nausea / vomiting; heartburn);
    nausea and vomiting of a functional, infectious, organic origin caused by drug therapy, radiotherapy or a dietary disorder, as well as dopamine agonists if used in Parkinson's disease (e.g., levodopa and bromocriptine).


Contraindications

    prolactinoma (prolactin-secreting pituitary tumor);
    mechanical intestinal obstruction, perforation of the stomach or intestines, in which stimulation of the motor function of the stomach can be dangerous;
    gastrointestinal bleeding;
    glucose-galactose malabsorption or lactase deficiency, hereditary lactose intolerance;
    age up to 5 years and weight of the child less than 20 kg;
    high sensitivity to the components of the drug.

Instructions for use Domstal: method and dosage

Tablets are taken orally 15-30 minutes before a meal.
Recommended dosage for children older than 5 years and adult patients: 3 mg per day, 10 mg each, if necessary, an additional 10 mg at bedtime. In cases of severe vomiting / nausea or severe dyspepsia, 3-4 mg per day, 20 mg each.
Patients with renal failure should reduce the frequency of administration of the drug.

Side effects

    digestive system: transient intestinal cramps (completely disappear after discontinuation of treatment);
    central nervous system: extrapyramidal disorders in patients with increased permeability of the blood-brain barrier and in children (they are transient in nature and pass independently after treatment); it is impossible to completely eliminate the likelihood of such side effects due to the underdevelopment of the blood-brain barrier (for example, in children) or its high permeability;
    hypersensitivity reactions: urticaria and rash;
    other reactions: hyperprolactinemia (galactorrhea, gynecomastia).

Overdose

In case of an overdose, the following symptoms can be observed (especially in children): disorientation, extrapyramidal disorders, drowsiness.
For therapy, the use of activated carbon and careful monitoring of the patient's condition is recommended. In the event of extrapyramidal reactions, anticholinergics, drugs used for parkinsonism, or antihistamines with an anticholinergic effect may be effective.

Special instructions

In the case of complex use of the drug Domstal with antisecretory or antacid agents, the latter must be taken after meals. At the same time, Domstal is taken before meals, as the absorption of domperidone slows down a bit when there is food in the stomach.

Pregnancy and lactation

Information on the use of the drug during pregnancy is not enough.
There is currently no evidence of an increased risk of developing malformations in humans. Domstal should be prescribed only if the expected benefit to the mother outweighs the potential risk to the child.
The concentration of domperidone in breast milk is 4 times lower than in plasma. Whether this level contributes to adverse effects on newborns is not known. When using the drug, breastfeeding should be discontinued.

Use in childhood

The use of the drug is contraindicated in children under 5 years of age.

With impaired renal function

In renal failure, it is recommended to reduce the frequency of administration of the drug.

With impaired liver function

With severe liver failure, Domstal should be used with caution.

Drug interaction

    azole-type antifungal drugs, HIV protease inhibitors, macrolide antibiotics, nefazodone (antidepressant): increased plasma domperidone concentration;
    anticholinergics: possible neutralization of the action of Domstal;
    cimetidine, sodium bicarbonate: there is a decrease in their bioavailability;
    antipsychotics: domperidone does not enhance their effect;
    agonists of dopaminergic receptors (bromocriptine, levodopa): suppression of undesirable peripheral effects, such as nausea, vomiting, digestive disorders, without neutralizing their basic properties;
    paracetamol and digoxin: the concentration of these drugs in the blood does not change.
