Description

Dexilant is a drug that suppresses the secretion of digestive juice, delaying the last stage of hydrochloric acid production. The capsule of the medication breaks down in the stomach, releasing acid-resistant granules. The active substance leaves them at a suitable pH in different parts of the small intestine. This combination gives a prolonging effect, reducing the release of gastric juice for a long period.

Mechanism of action

The active component of Dexilant is easily absorbed after taking. It has high bioavailability, which reaches 76% or more. The combination of two components provides absorption in different pH phases. The maximum accumulation of the active component appears in the first hour - two from the time of adoption, the second release period begins in the range of 4 - 5 hours. After 5 days of using the drug, the highest concentration of the drug in serum and blood reaches 658 - 1397 ng / ml.
Connection with plasma proteins reaches 96.1 - 98.8%.
The active component is intensively processed by the liver, to inactive substances, due to oxidative and reducing processes with the formation of sulfates, glucuronides and glutathione compounds.
Oxidative processes occur with the help of cytochrome enzymes, which take part in hydroxylation and oxidation.
The period of time for which the concentration of the drug is reduced by half in the body takes 1-2 hours.
Complete cleansing of the body after 5-day treatment occurs at 11.4 and 11.6 l / h in accordance with the dosage.
The drug is excreted through the kidneys (up to 51%), and 48% removes the intestines.
Due to the fact that dexlansoprazole is processed by the liver, patients with impaired renal function do not adjust the dose of Dexilant.

Dexilant used with HIV protease inhibitors reduces their bioavailability, due to the dependence of absorption on the pH of the stomach.
Extremely carefully prescribed for use by patients undergoing therapy with Tacrolimus, inhibitors of the CYP2C19 isoenzyme - Fluvoxamine, Methotrexate and Warfarin, conducting mandatory monitoring of the prothrombin period and INR.

Method of administration, dosage

The action of Dexilant with other medications

A pharmaceutical can be prescribed to patients undergoing clopidogrel therapy without additional dosage adjustment. No effect or interaction was recorded with the concomitant use of Diazepam, Theophylline, Phenytoin with a medication.
Taking dexlansoprazole capsules disrupts the absorption of medications that are dependent on the pH of the stomach, such as iron salts, Ampicillin, Digoxin, Ketoconazole, Erlotinib esters.
Taking it in tandem with Tacrolimus in patients after organ transplantation contributes to the accumulation of Tacrolimus in plasma.
Concurrent administration of dexlansoprazole and fluvoxamine increases the likelihood of a systemic effect of the former.
The use of Dexilant with methotrexate leads to the growth and conservation of a high percentage of accumulation of methotrexate and its residues in blood plasma. In need of high-dose therapy, the instruction recommends temporarily stop the use of dexlansoprazole.

Pregnancy, lactation

Therapy of pregnant women with this medicine is not recommended. If treatment with a drug during the lactation period is necessary, then lactation should be completed.

For children under the age of 18 Dexilant is not prescribed.

Special instructions

Before you start using Dexilant, you need to make sure that there are no cancerous tumors, because it hides the signs and complicates the establishment of a residual diagnosis.

Dexlansoprazoles increase the risk of digestive tract infections caused by the bacterium Clostridia difficile, and are accompanied by diarrhea. This should be taken into account if diarrhea therapy does not give positive results.
Therefore, patients are prescribed a dose in minimally effective concentrations in short courses.
In patients with the recommended maximum doses with prolonged treatment with dexlansoprazole, there is a chance of fracture of the tibia, spinal column, and brushes throughout the year. People with this predisposition should not violate appointments.
