Description of the drug

The controloc (controloc) is a medication, a proton pump inhibitor. The active substance of the described drug is pantoprazole. The instructions for use of Controloc indicate that the drug is used for short-term therapy of signs of acid reflux in adults. Acid reflux is a process during which acid produced in the stomach enters the esophagus, causing heartburn and acid regurgitation (a condition where acid enters the mouth from the stomach). In addition to treating heartburn, the medicine is intended for long-term therapy of a wider range of conditions that affect the intestines. As an analogue of Controloc in Moscow, one can name such medicines as Nolpaza, Pantaz, Sanpraz, Panumi and others. All named medications are registered in Russia. The medicine can be obtained without a prescription. You can buy Controloc without a prescription. Price Controloc starts from about 300 rubles. To purchase Controloc tablets in Moscow, place your order in an online pharmacy with delivery in Russia, including by courier to the door of your apartment or house. The drug has positive reviews among patients receiving it.

Release form, composition and packaging

There are three drug configurations: a solid dosage form of 20 mg and 40 mg, and powder placed in vials, injected through a vein in the solution. The drug is produced in the form of oval yellow solid dosage forms in a dissolving coating. On tablets there is an inscription P20 / P40, which indicate the dosage. One tablet contains 22.6 mg and 45.1 mg of pantoprazole sodium sesquihydrate, which corresponds to 20 mg and 40 mg of the drug, respectively. Additional components are: • povidone K90; • anhydrous soda ash or Na2CO3; • osmotic diuretic - mannitol; • calcium stearic acid; • crospovidone; • Purified water.

Pharmachologic effect

Pantoprazole works by preventing the proteins (“proton pumps”) found in specialized cell cells in the stomach lining from pumping acid contents into the stomach. By inhibiting the work of the proteins involved in pumping acid, the drug lowers acid production, eliminating signs of acid reflux. When a drug is in an acidic environment, it begins to work actively, reducing emissions of hydrochloric acid in its final stage (H + / K + -ATPase), regardless of what the nature of the irritating substance is. When pantoprazole is used, basal and stimulated secretions of gastric juice are suppressed, Ph is reduced and gastrin is formed. If you drink pantoprazole for a short time, then the indications of gastrin are kept within the upper norm. If therapeutic therapy is carried out for a long time, then gastrin indices can double. If the drug is taken for a long time, a slight increase in the number of endocrine gastric cells may be noticeable. These manifestations are quite rare. The medication is rapidly absorbed after 2-2.5 hours, the concentration is 1-1.5 μg / ml, and the Controloc is eliminated half an hour later. The Controloc with the participation of the enzyme system of cytochrome P450 is actively cleaved in the liver. Part of the drug is extracted through the kidneys, a small portion is excreted in the feces. After application it is assimilated in full. Renal disorders, the passage of the hemodialysis process can not be called the reasons for lowering the dose. The Controloc from the diseased organism is eliminated as quickly as in a healthy person. Pantoprazole is not removed during hemodialysis. A medication does not change the motor function of the digestive tract. With a duodenal ulcer associated with Helicobacter pylori, gastric secretion decreases, which causes an increase in sensitivity to antibacterial drugs. Density and bioavailability are not directly related to food intake. Comparing with drugs of the same action, the drug is chemically more stable if the acidity is neutral.

Indications

The Controloc is indicated for: • reflux esophagitis (a digestive system disorder in which food containing gastric juice enters the esophagus); • peptic ulcer of the stomach and duodenum in the acute phase; • erosive gastritis (also associated with the use of NSAIDs); • Zollinger-Ellison syndrome (a pancreatic tumor that contributes to the creation of a large volume of gastrin, which promotes the production of hydrochloric acid by the stomach); • Helicobacter pylori is destroyed along with antibacterial drugs.

Contraindications

Pantoprazole is forbidden to use in case of digestive disorders due to stress, pregnancy, and breastfeeding. The combined use of pantoprazole and atazanavir (a drug used in the treatment of HIV infection) is contraindicated. If the use of atazanavir together with Controloc is an urgent need, then clinical observation should be carried out in combination with an increase in the dosage of atazanavir to 400 mg with 100 mg of ritonavir. An amount of pantoprazole of 20 mg / day should be observed. Controloc are prohibited for minors and people with acute sensitivity to soy and to the ingredients of the drug.

Dosage

The instruction will allow you to understand how to take the Controloc. The medicine in tablet form 20 mg is used to get rid of signs of heartburn, nausea, acid belching. To relieve symptoms in a patient, you need to use the drug for 2-3 days. To completely get rid of the manifestations of the disease, you may need to increase the duration of therapy to seven days. If the condition continues to worsen, then you need to consult a doctor. If the painful manifestations have disappeared, you need to stop using the medication. If there is no improvement, seek the help of a healthcare professional. A tablet medicine of 40 mg or more is used to eliminate the manifestations of: • gastric ulcer and 12 duodenal ulcer in the acute phase; • erosive gastritis (also associated with the intake of NVSP); • Zollinger-Ellison syndrome; • eradication Helicobacter pylori (Hp) in conjunction with antibacterial drugs. The Controloc in tablet form should be swallowed and washed down with water. It is forbidden to break into smaller parts or chew. The powdered form of the drug is used when it is difficult to swallow. If ingestion is difficult, then intravenous administration of pantoprazole at a dosage of 40 mg / day is possible. Solution preparation and drug administration should only be done by medical personnel. To make the composition: • add 10 ml of 0.9% saline solution of sodium chloride to the powder; • dilute the resulting mixture with 100 ml of 0.9% sodium chloride or with a five percent glucose solution. The resulting drug mixture can be used for another 3 hours, but it is recommended to enter a maximum of 15 minutes. The acidity of the mixture for intravenous infusion should be equal to 9. Controloc in powder form cannot be diluted with solvents other than those indicated. In order to prevent the onset of stomach diseases, it is not advisable to drink a medication.

Side effects

When using the medication according to the prescribed dosage and frequency of use, side effects are rarely detected. Frequent (approximately in 1 patient out of 10) side effects can be headaches, indigestion. Side effects ranked by frequency classification: • very often (≥1 / 10); • general (from 1/100 to less than 1/10); • rarely (from ≥1 / 1000 to less than 1/100); • rarely (from 1/10000 to less than 1/1000); • very rarely (1/10000); • unknown (cannot be estimated). For all the consequences of administration, it is impossible to apply any frequency of side effects, so an “unknown” frequency is assigned. Side effects are placed in decreasing order of frequency. The likely consequences of taking the medication can be the following: • for the hematopoietic system: agranulocytosis, thrombocytopenia, leukopenia, pancytopenia; • for the nervous system: headaches, dizziness, etc .; • for organs of vision: blurred vision; • for the gastrointestinal tract: diarrhea, bloating, nausea, possibly with vomiting, flatulence, constipation; • for the kidneys and urinary tract: interstitial nephritis; • for skin and subcutaneous tissues: itching, exanthema or rash, urticaria, angioedema, malignant exudative erythema, etc. • for the musculoskeletal system: myalgia, arthralgia; • for metabolism: hyperlipidemia, increased lipid concentration, changes in body weight, etc .; • for the immune system: anaphylactic reactions and anaphylactic shock; • for the psyche: sleep disturbance, depression with exacerbation of already diagnosed disorders, hallucinations; • for the reproductive system and mammary glands: there is a chance of gynecomastia. Therapy with the inclusion of pantoprazole causes a slight increase in the risk of FA infections caused by bacteria, for example, salmonella, campylobacter, clostridium difficile. General ailments, also fever, peripheral swelling are possible.

Overdose

No overdose events have been observed. If there are clinical manifestations and an overdose has occurred, symptomatic therapy should be prescribed. With an intravenous infusion of up to 2400 mg for two minutes, no fatal effects were found.

Drug interaction

If the bioavailability of the drug is directly dependent on the level of acidity of the stomach, then with the combined therapy with pantoprazole, the absorption of certain drugs will be difficult. For example, azole antifungal medicines: ketoconazole, itraconazole, etc. The combined use of Controloc and atazanavir, the absorption of which is determined by the level of acidity, helps to reduce the bioavailability of the drug for HIV. This will likely affect the therapeutic effect of these medicines. Pantoprazole can be taken in conjunction with medications metabolized with the participation of cytochrome 450, for example, phenazepam, diazepam, theophylline, carbamazepine, diclofenac, naproxen, piroxicam, phenytoin, ethanol. Clinically important indicators are not fixed during the joint use of the medication with substances containing ethanol, caffeine, theophylline. Conducted studies of co-administration of pantoprazole with antibiotics (clarithromycin, metronidazole, amoxicillin) did not reveal important facts. Does not affect the level of effectiveness of hormonal contraception.

Special instructions

If there is a malignant formation, pantoprazole will mask the symptoms, therefore, before use, it is necessary to verify the absence of this formation. A patient must be visited by a qualified healthcare professional if a urea breath test or endoscopy is planned. Also, medical consultation is necessary for: • unexpected weight loss; • anemia; • LCD bleeding; • swallowing disorder, vomiting, since the use of a medication harms the correct diagnosis, temporarily eliminating the symptoms of the disease; • gastrointestinal surgery; • liver disease; • the presence of liver failure; • long-term symptomatic (more than 4 weeks) therapy to eliminate heartburn and dyspepsia; • other diseases affecting the general condition of a person; • if the patient is over 55 years old. Since the drug affects vision and can cause dizziness, it is recommended to abandon driving a car or other vehicle at the time of admission.

Pregnancy and lactation

The medication is harmful if you use it during pregnancy and while breastfeeding. The use of pantoprazole is permissible only when the beneficial effect for the mother is much greater than the potential danger to the unborn child. Contraindication during lactation is caused by the release of pantoprazole into breast milk.

Use in childhood

The drug is forbidden to use in the treatment of children and adolescents until adulthood.

With impaired renal function

Patients with malfunctioning kidneys, and people after 60 years are required to take medicine in an amount of 40 mg per day. A departure from the rule may be mixed therapy directed against Helicobacter pylori. This means that elderly patients can take the medicine twice a day at 40 mg.

With impaired liver function

Patients who have problems with the liver, need to take a medication in a dose of 40 mg once every two days. It is also necessary to monitor blood biochemistry. If the level of liver enzymes increases, you need to finish taking the medication.

Use in old age

Patients who have overcome 60 years of age should be monitored so that the daily dose of the drug is not more than 40 mg. An exceptional case can be called mixed antibacterial treatment against Helicobacter pylori, when pantoprazole is used twice a day for 20-40 mg.
