Bromhexin 8 - a drug with mucolytic and expectorant action.

Release form and composition

The dosage form for the release of Bromhexin 8 is a dragee: from greenish-yellow to yellow, biconvex, the core is almost white (20 or 25 each in blisters, 1 or 2 blisters are placed in a cardboard bundle).
Composition of 1 tablet:
    active substance: bromhexine hydrochloride - 8 mg;
    auxiliary components: lactose monohydrate - 34.4 mg; magnesium stearate - 0.6 mg; corn starch - 14.6 mg; gelatin - 1.8 mg; colloidal silicon dioxide - 0.6 mg;
    shell: sucrose - 27,704 mg; calcium carbonate - 4.326 mg; macrogol 6000 - 1.75 mg; glucose syrup - 1,639 mg; magnesium carbonate - 1.507 mg; talc - 1.507 mg; titanium dioxide (E 171) - 1.166 mg; povidone K-25 - 0.243 mg; quinoline yellow (E 104) - 0.146 mg; carnauba wax - 0.012 mg.

Pharmacodynamics

Bromhexine has an expectorant and mucolytic effect.
It activates the ciliated epithelium, reduces the viscosity of sputum, increases the volume of sputum and improves its discharge, and also stimulates the production of endogenous surfactant, which ensures the stability of alveolar cells during respiration.
The therapeutic effect develops after 2–5 days from the start of administration.

Pharmacokinetics

When taken orally, 99% of bromhexine is absorbed from the gastrointestinal tract for 30 minutes, and communication with plasma proteins is 99%.
Bromhexine crosses the blood-brain and placental barriers and into breast milk.
It undergoes oxidation and demethylation in the liver, is metabolized to ambroxol. In severe liver diseases, a decrease in clearance of bromhexine is noted.
The half-life is 15 hours (this is due to slow back diffusion from the tissues). It is excreted in the form of metabolites by the kidneys.
In patients with chronic renal failure, the allocation of metabolites is impaired.
It can accumulate with repeated use.

Indications for use

According to the instructions, Bromhexin 8 Berlin-Chemie is used for sputum secretion and transport disorders associated with acute / chronic bronchopulmonary diseases (tracheobronchitis, pulmonary emphysema, acute and chronic bronchitis, etc.).

The life-threatening effects of an overdose are unknown.
Possible symptoms: dyspeptic disorders, nausea, diarrhea, vomiting.
Therapy: gastric lavage, artificial vomiting for the first 1-2 hours after taking the drug.
With forced diuresis or hemodialysis, elimination of bromhexine does not occur (due to the high volume of distribution and a high degree of protein binding).

special instructions

In the process of therapy, it is recommended to drink a sufficient amount of liquid, since this increases the secretolytic effect of the drug.
Due to the risk of delayed discharge in the airways in case of bronchial motility disorders or a large amount of sputum excreted, taking Bromhexin 8 Berlin-Chemie requires caution.
In patients with severe renal failure, the possibility of cumulation of metabolites formed in the liver should be considered. During prolonged therapy, monitoring of renal function is recommended.

Pregnancy and lactation

I trimester of pregnancy is a contraindication to taking the drug.

Use in childhood

Children under 6 years of age Bromhexin 8 Berlin-Chemie is not assigned.

In case of impaired renal function

The appointment of the drug in renal failure requires caution.

With impaired liver function

Drug interaction

Bromhexin 8 Berlin-Chemie should not be used in combination with antitussive drugs (including codeine), since this leads to difficulty in evacuating liquefied sputum.
In the first 4–5 days of antimicrobial therapy, bromhexine promotes the penetration of cephalexin, erythromycin, ampicillin, oxytetracycline and other antibacterial drugs into bronchial secretions.
