Indications for use

Arterial hypertension treatment:
- in patients whose blood pressure is insufficiently controlled by telmisartan or amlodipine, used as monotherapy;
- in patients for whom combination therapy is indicated;
- in patients receiving telmisartan and amlodipine in separate dosage forms, as a replacement for this therapy.

Active ingredient, group

Amlodipine + Telmisartan, Antihypertensive combined agent (angiotensin II receptor blocker + BMCC).

Contraindications for Telsartan AM

- obstructive diseases of the biliary tract;
- severe arterial hypotension;
- obstruction of the outflow tract of the left ventricle (including a high degree of aortic stenosis);
- hemodynamically unstable heart failure after acute myocardial infarction;
- severe liver failure;
- shock;
- simultaneous use with aliskiren in patients with diabetes mellitus or impaired renal function (GFR less than 60 ml / min / 1.73 m2);
- fructose intolerance and glucose / galactose malabsorption syndrome or sucrase / isomaltase deficiency;
- pregnancy;
- lactation period;
- age up to 18 years (efficacy and safety have not been established);
- hypersensitivity to active ingredients or excipients;
- hypersensitivity to other derivatives of dihydropyridine.
Telsartan AM should be prescribed with caution in case of impaired liver function, renovascular arterial hypertension, primary aldosteronism, stenosis of the aortic and mitral valves, hypertrophic obstructive cardiomyopathy, heart failure, diabetes mellitus, unstable angina pectoris, acute myocardial infarction, impaired renal function, condition after BCC and / or hyponatremia, hyperkalemia, and other conditions characterized by activation of the RAAS.

How to use Telsartan AM: dosage and course of treatment

Telsartan AM is taken orally 1 time / day, regardless of food intake.
Treatment of hypertension in a patient can be started with the use of Telsartan AM in the case when it is assumed that it is unlikely to achieve blood pressure control with any one drug. The recommended initial dose of the drug is 40 mg / 5 mg 1 time / day. In patients who need a more significant decrease in blood pressure, the initial dose of the drug can be increased to 80 mg / 5 mg 1 time / day.
If, after at least 2 weeks of treatment with Telsartan AM, an additional decrease in blood pressure is required, the dose of the drug can be gradually increased to the maximum - 80 mg / 10 mg 1 time / day.
Can be used in conjunction with other antihypertensive drugs.

Pharmachologic effect

An antihypertensive combination drug (angiotensin II receptor blocker + BMCC), a combined antihypertensive drug containing two active components with a complementary effect, allowing to control blood pressure in patients with arterial (essential) hypertension: an angiotensin II receptor antagonist (telmisartan) and a slow channel blocker dihydropyridine (amlodipine). The combination of these substances has an additive antihypertensive effect, lowering blood pressure to a greater extent than each component alone. The drug Telsartan AM, taken 1 time / day, leads to an effective and sustained decrease in blood pressure within 24 hours.

Stevens-Johnson Syndrome.

Special instructions

During treatment with drugs that affect the RAAS, especially in the presence of impaired renal function and / or heart failure, hyperkalemia may occur. In such patients, regular monitoring of serum potassium is recommended. Periodic monitoring of serum creatinine concentration is also recommended for patients with impaired renal function.

Interaction

With simultaneous use Telsartan AM with other antihypertensive drugs, the hypotensive effect of the drug may increase.
Based on the experience of using other drugs that affect the RAAS, the simultaneous use of the drug and potassium-sparing diuretics, potassium-containing supplements, potassium-containing food salt, and other drugs that increase the level of potassium in the blood (for example, heparin) can lead to hyperkalemia. Therefore, the use of such combinations requires caution and control of the potassium content in the blood.
