Nitrocor is a drug used to relieve angina attacks.

Release form and composition

The dosage form of the release of Nitrocor is sublingual tablets: flat-cylindrical, white or almost white, with a chamfer (in a cardboard box 1 can of 40 tablets).
Composition of 1 tablet:
    active substance: nitroglycerin - 0.5 mg (nitroglycerin with lactose);
    auxiliary components: potato starch, stearic acid, colloidal silicon dioxide, croscarmellose sodium salt, aspartame, microcrystalline cellulose.

Pharmacodynamics

Nitrocor is a peripheral vasodilator that mainly affects the venous vessels. The action of nitroglycerin is mainly associated with a decrease in myocardial oxygen demand (due to a decrease in preload - expansion of peripheral veins and a decrease in blood flow to the right atrium, and afterload - a decrease in total peripheral vascular resistance).
It has a central inhibitory effect on the sympathetic vascular tone, inhibits the vascular component of the formation of pain syndrome. It leads to the expansion of the meningeal vessels (this is the cause of headaches when using it). An attack of angina pectoris when Nitrocor is applied under the tongue, as a rule, stops after 1.5 minutes, the time for maintaining the antianginal and hemodynamic effect is in the range of 30-60 minutes.

Pharmacokinetics

Nitroglycerin from the surface of the mucous membranes is absorbed completely and quickly. When the drug is taken under the tongue, the substance immediately enters the systemic circulation, its bioavailability is 100%.
The maximum plasma concentration in the blood is reached after 5 minutes.

Contraindications

Absolute:
    lactose intolerance, lactase deficiency, glucose-galactose malabsorption syndrome;
    shock;
    phenylketonuria;
    collapse;
    age under 18;
    individual intolerance to any of the components of the drug.
Relative (Nitrocor is prescribed under medical supervision after assessing the benefit / risk ratio):
    combination therapy with phosphodiesterase 5 inhibitors (sildenafil, tadalafil, vardenafil, etc.);
    cranial hypertension;
    uncontrolled hypovolemia;
    cardiac tamponade;
    heart failure with low / normal pulmonary artery pressure;
    arterial hypotension (in patients with systolic blood pressure less than 90 mm Hg. Art.);
    angle-closure glaucoma with high intraocular pressure;
    violation of cerebral circulation;
    subarachnoid hemorrhage;
    severe hepatic / renal impairment (due to the risk of methemoglobinemia);
    severe anemia;
    toxic pulmonary edema;
    hyperthyroidism;
    idiopathic hypertrophic subaortic stenosis;
    a recent head injury;
    hemorrhagic stroke;
    conditions accompanied by a decrease in the filling pressure of the left ventricle (acute myocardial infarction, isolated mitral stenosis, constrictive pericarditis);
    pregnancy and lactation.


Instructions for the use of Nitrocor: method and dosage

Before using Nitrocor, you should consult your doctor.
The drug should be taken at the first sign of an angina attack.
Recommended dosage regimen: 1 tablet under the tongue (do not swallow, until completely dissolved). Repeated administration of the drug in the same dose is possible in cases when the angina attack is not stopped after 5 minutes. If, after taking a repeated dose, the therapeutic effect is insufficient / absent, it is necessary to seek medical help.
For patients with frequent attacks of angina pectoris, it is advisable to use drugs of the nitroglycerin series with prolonged action.

Side effects

    nervous system: disorientation, blurred vision, nitrate headache (especially at the beginning of the course of treatment, the severity of the disorder decreases with continued therapy), lethargy, feeling of weakness, dizziness, psychotic reactions, anxiety;
    cardiovascular system: lowering blood pressure, tachycardia; rarely (mainly in case of overdose) - redness of the facial skin, cyanosis, orthostatic collapse;
    digestive system: xerostomia, vomiting, nausea, abdominal pain;
    others: hypothermia, allergic reactions.

Overdose

The main symptoms: headache, severe dizziness, fainting, a significant decrease in blood pressure (below 90 mm Hg) with orthostatic dysregulation, asthenia, convulsions, drowsiness, shortness of breath, palpitations, severe weakness, fever; when using doses greater than 0.02 mg / kg - tachypnea, collapse, methemoglobinemia, cyanosis, dyspnea.
Therapy: the patient should be laid down with raised legs and urgently seek medical help.


Special instructions

While taking Nitrocor, there may be a significant decrease in blood pressure and dizziness with a sharp transition from a sitting or lying position to an upright position, as well as in hot weather, when exercising and combined with alcohol.
If xerostomia or blurred vision is severe or persists, therapy is discontinued.
With frequent use, addiction may develop, which requires an increase in the dose.
It is possible to reduce the severity of headache while taking Nitrocor by reducing its dose or by taking validol at the same time.

Influence on the ability to drive vehicles and complex mechanisms

Patients when driving motor vehicles should take into account the likelihood of a decrease in mental and motor response.

Application during pregnancy and lactation

According to the instructions, Nitrocor can be prescribed to women during pregnancy / lactation solely for health reasons.

Childhood use

For patients under the age of 18, therapy is contraindicated.

With impaired renal function

Drug interactions

Possible interactions:
    vasodilators, antihypertensive drugs, angiotensin-converting enzyme inhibitors, phosphodiesterase inhibitors, slow calcium channel blockers, tricyclic antidepressants, alcohol, diuretics, beta-blockers, monoamine oxidase inhibitors, procainamide (uncontrolled antigenic action)
    dihydroergotamine: an increase in its content in the blood and an increase in blood pressure (associated with an increase in the bioavailability of dihydroergotamine);
    heparin: a decrease in its effectiveness (after discontinuation of the drug, blood clotting may significantly decrease).
