Instruction for Monocinque Retard

You can buy Monocinque Retard here

The antianginal drug is manufactured by the German biopharmaceutical company Berlin Chemie. It is an organic nitrate, the pharmacological action of which extends mainly to the venous vessels. Course use of the drug helps to reduce preload and afterload on the heart muscle. A decrease in the oxygen demand of muscle fibers leads to the elimination of the main symptoms of myocardial hypoxia.

Dosage form

Monocinque retard is available in the form of small pinkish-orange hard gelatin tablets. They include the following components:
    isosorbide mononitrate 50.0 mg - the main active ingredient;
    sucrose
    corn starch
    ethyl cellulose
    stearic acid, talc, dye red 4R (E124), quinoline yellow (E104), titanium dioxide, gelatin.
A cardboard box contains 30 tablets packed in PVC and aluminum foil blisters.

Pharmacotherapeutic properties

The cardiological drug Monocinque Retard has pronounced antianginal and vasodilating properties. Isosorbide mononitrate affects the tone of the venous vessels. When the tablets are taken, nitric oxide is synthesized in the capillary walls, due to which the production of intracellular guanylate cyclase is activated. In this regard, the concentration of the vasodilation mediator increases.
Monocinque retard reduces the demand of the heart muscle for oxygen molecules, which is due to a decrease in preload and afterload. With a decrease in blood flow to the right atrium, the pressure in the vessels from the pulmonary circulation decreases. Redistribution of coronary blood microcirculation increases the mechanical resistance of the myocardium to stress in patients with coronary artery disease.

Indications for use

Peripheral vasodilator is recommended for people with coronary artery disease. It restores the blood supply to the myocardial tissue, which reduces the risk of angina pectoris. Indications for the appointment of Monocinque retard 20 are:
    postinfarction condition;
    chronic heart failure;
    prevention of angina pectoris;
    coronary artery disease.
The vasodilator can be used in the complex therapy of cardiological diseases such as obliterating endarteritis, hypertension in the pulmonary circulation, angiospastic retinitis, etc.

Dosage regimen

The dosage is determined in accordance with the patient's anamnestic data and the characteristics of the pathology. The maximum dose of a vasodilator is 60 mg / day. Typically, therapy is started at 10 mg / day, i.e. ½ tablet per day. You need to drink the medicine with still water.
Over time, the dosage is increased, but only on the recommendation of a cardiologist and with parallel ECG monitoring. At least 8 hours should pass between pills. The minimum duration of therapy is 2-3 weeks.

Pregnancy and lactation

The use of Monocinque retard tablets during gestation is possible only on the recommendation of the attending physician. In case of an overdose, the penetration of isobride mononitrate into the child's body is possible, which entails an abnormal development of the cardiovascular system.
In taking pills Monocinque Retard during the lactation period, you need to transfer the newborn to artificial feeding. Nitrates are excreted into milk, therefore, they can provoke the development of methemoglobinemia in a child.

Alcohol compatibility

Alcoholic drinks increase the hypotensive activity of Monocinque Retard 20, which often causes nausea, muscle weakness, drowsiness and decreased appetite.
Interaction with medications
Simultaneous administration of hypotensive patients enhances the antihypertensive activity of the drug. In this regard, during the course of treatment, it is undesirable to take beta-blockers, tricyclic antidepressants and antipsychotics.

Overdose and adverse reactions

According to the instructions for use, an overdose of vasodilating tablets is fraught with a strong deterioration in well-being:
    increased sweating;
    hyperthermia;
    fainting;
    hyperapnea;
    dizziness;
    visual disturbances;
    bradycardia;
    convulsions;
    collapse.
To reduce the severity of signs of overdose, you need to rinse the stomach. When methemoglobinemia appears, parenteral administration of ascorbic acid is prescribed.
The instruction warns of a rather high probability of side effects during therapy:
    skin rashes;
    increased heart rate;
    collaptoid states;
    hyperemia of the skin;
    dyspeptic symptoms;
    exfoliative dermatitis.
Therapy should be discontinued if tachycardia develops and angina attacks intensify.

Contraindications

The cardiological agent is not recommended for children due to the high likelihood of side effects. Contraindications to the appointment of tablets will be:
    intolerance to galactose;
    constrictive pericarditis;
    aortic stenosis;
    severe anemia;
    cardiac tamponade;
    acute circulatory disorders;
    cerebral hemorrhage;
    arterial hypotension;
    hypertrophic cardiomyopathy.
Do not use the medicine if you are prone to allergies and hypersensitivity to nitro compounds.

Storage conditions

The tablets are stored for no more than 5 years, subject to the optimal temperature regime of 18-25 degrees Celsius.
