Medicinal composition and release form

The cardiological preparation Erinit is available in the form of small biconvex tablets for sublingual use. They include the following components:
    pentaerythrityl tetranitrate (10.0 mg);
    dextrose;
    food additive E1442;
    emulsifier E553b;
    sucrose;
    octadecanoic acid.
Tablets are packed in PVC cell plates, 10 pieces each. The pack contains 5 blisters along with instructions for the use of the angioprotective agent.

Therapeutic action

The medicinal drug has pronounced vasodilating (vasodilating) and antianginal properties. The mechanism of action of pentaerythrityl tetranitrate is to stimulate the release of dinitrogen oxide from the muscle fibers of blood vessels, which has a muscle relaxant effect on the walls of large veins.
When using Erinit, the pressure in the pulmonary circulation and blood flow to the right ventricle are reduced. This leads to a decrease in the pre- and afterload on the heart muscle, as well as a decrease in myocardial tissue oxygen demand. The therapeutic effect of the vasodilator also extends to the blood flow, in particular, when taking the pills, the blood supply to the ischemic areas of the heart is normalized.
In the case of systematic use of an angioprotective agent, it reduces the likelihood of exacerbation of angina pectoris and increases the resistance of the myocardium to excessive loads. Pentaerythrityl tetranitrate acts more slowly than nitroglycerin, but the vasodilating effect persists for at least 3-5 hours.

Indications for use

Most often, Erinit is used to prevent attacks of coronary artery disease and cardiac complications. Also, angioprotective tablets are used in polytherapy of the following cardiovascular pathologies:
    rehabilitation in the post-infarction period;
    violation of the contractile function of the myocardium;
    congestive heart failure;
    prevention of exacerbations of angina pectoris.
As part of combined pharmacotherapy, a venous vasodilator is prescribed for patients with coronary insufficiency. Tablets can be used during surgery to reduce pressure in peripheral vessels and, as a result, reduce blood loss.

Dosage regimen

The tablets are taken orally or sublingually (sublingually) 60 minutes before meals. The dosage regimen is determined by the degree of myocardial dysfunction and the severity of IHD symptoms:
    rehabilitation after myocardial infarction - 2 tablets 3-4 times a day;
    prevention of angina pectoris - 20 mg 15-20 minutes before bedtime;
    exacerbation of ischemic heart disease - 40 mg 4 times a day.
The duration of the course depends on the patient's well-being and therefore is determined by the cardiologist. In most cases, pharmacotherapy continues for 15-30 days. If necessary, the treatment is repeated, but only on the recommendation of a doctor.

Special instructions

The angioprotective agent is not used in pediatrics due to the high risk of collapse and hypotonic crisis. In this regard, Erinit is not prescribed for the treatment of cardiopathologies in patients under the age of 18.
Due to the slow development of the vasodilating effect, tablets are not used to relieve exacerbations of angina pectoris. The cardiological preparation is suitable for the prevention of exacerbations of coronary artery disease and for reducing the severity of symptoms of myocardial insufficiency.
The tablets contain sucrose and dextrose, which should be taken into account when using the medicine for patients with diabetes mellitus. To prevent hyperglycemia, it is recommended to increase the daily dose of insulin. When undergoing pharmacotherapy, it is necessary to monitor the functions of vital organs and hemodynamic blood parameters.
With prolonged use of an angioprotective agent, tolerance to pentaerythrityl tetranitrate often develops. To achieve the desired effect, the dosage is increased or Erinit is replaced by other means of vasodilating and antianginal action.

Gestation and lactation

An angioprotective agent is not used to treat cardiovascular disease in patients during gestation and breastfeeding. The use of tablets in the first trimester of pregnancy is fraught with intrauterine fetal malformations, and in the third - weakening of labor and the complete cessation of contractions during childbirth.

Alcohol compatibility

Alcoholic beverages potentiate the vasodilating effect of pentaerythrityl tetranitrate and, as a result, the development of a hypotonic crisis. Therefore, when using an antianginal drug, alcohol is not recommended.

Interaction with medications

Before combining Erinit with other medicines, you should consult your doctor. Under the influence of dihydroergotamine, the antianginal activity of Erinit decreases. When taking antihypertensive drugs, the risk of a critical decrease in blood pressure and circulatory disorders in the brain increases.

Overdose

A single dose of more than 90 mg of pentaerythrityl tetranitrate is often accompanied by an exacerbation of the following side effects:
    dizziness;
    vomiting;
    excessive sweating;
    insufficient blood circulation in the brain;
    shallow breathing;
    mydriasis;
    feeling tired.
To prevent the development of orthostatic insufficiency, you need to rinse the stomach and take sorbents. If the patient's health worsens, it is recommended to lay the patient on the floor and raise his legs in order to improve the blood supply to the brain and heart.

Side effects of Erinit

When taking Erinit tablets, approximately 3.5% of patients develop side reactions from the vessels, nervous system, gastrointestinal tract and skin. The most significant undesirable effects include:
    migraine;
    rapid fatigue;
    feeling of oversaturation in the stomach;
    vomiting;
    decreased appetite;
    unpleasant taste in the mouth;
    increased sweating;
    erythematous spots on the skin;
    unreasonable anxiety;
    orthostatic hypotension;
    hypotonic crisis;
    tachycardia;
    stool disorders;
    noise in ears;
    spatial disorientation.
Skin reactions are most often caused by hypersensitivity to the active and inactive components of the cardiac drug. Most of the side effects are mild and disappear within 2-3 days after starting therapy. If your health condition worsens, you need to stop taking pills and consult a cardiologist.

Contraindications for use

An angioprotective agent is not prescribed for patients during gestation and lactation, as well as for persons under the age of 18. Also contraindications to taking vasodilating tablets are:
    severe hypotension;
    angle-closure glaucoma;
    hypersensitivity to nitrates;
    orthostatic hypotension;
    hypotonia of cerebral vessels;
    ulcerative lesion of the stomach;
    traumatic brain injury;
    hemorrhagic stroke;
    shock state;
    collapse;
    acute myocardial infarction;
    bleeding tendency;
    hemophilia;
    decreased platelet aggregation.
To prevent cross-allergic reactions and the development of tolerance to pentaerythrityl tetranitrate, it is recommended that an interval of at least 60-90 minutes be observed between taking several antihypertensive drugs. When bone marrow functions are suppressed, therapy is performed under constant control of hemodynamic blood parameters.

Storage conditions

Store tablets in a ventilated place away from fire at temperatures up to 25 degrees Celsius. Their shelf life is 36 months from the date of issue.
