Biol is a synthetic drug with an antihypertensive effect.

Release form and composition

Biol is produced in the form of film-coated tablets: round, with a light yellow shell, white inside, engraving "BIS" on one side and a number corresponding to the content of the active substance (2.5, 5 or 10), on the other - cross-shaped risk (in blister contour packs of 10 pcs., in a cardboard box 3, 5 or 10 packs).
Active ingredient: bisoprolol hemifumarate, in 1 tablet - 2.5 mg, 5 mg or 10 mg.
Excipients: calcium hydrogen phosphate, corn starch, anhydrous colloidal silicon dioxide, microcrystalline cellulose, croscarmellose sodium and magnesium stearate.

Indications for use

The active ingredient Biol has an antihypertensive effect without its own sympathomimetic activity. As a result of the use of the drug, the minute blood volume decreases, the activity of the sympathoadrenal system decreases and the myocardial demand for oxygen decreases.
The use of Biol is recommended for the treatment of the following diseases:
    Ischemic heart disease, including prevention of attacks of stable angina pectoris;
    Arterial hypertension;
    Chronic heart failure (as part of combination therapy).

Contraindications

    Acute heart failure and chronic heart failure in the stage of decompensation;
    Collapse;
    Sick sinus syndrome;
    Severe arterial hypotension;
    Severe peripheral circulatory disorders;
    Severe forms of bronchial asthma and chronic obstructive pulmonary disease;
    Bradycardia;
    Raynaud's syndrome;
    Hypersensitivity to the components of Biol and other beta-blockers;
    Pheochromocytoma;
    AV block II and III degree;
    Cardiogenic shock;
    Metabolic acidosis;
    Sinoatrial blockade;
    Cardiomegaly;
    Concomitant use of floktaphenin and sultopride.
Biol is not recommended to be taken under the age of 18.
The use of the drug during pregnancy can lead to intrauterine death of the fetus, slower growth, miscarriages or premature birth. There is a possibility of pathological reactions, such as intrauterine hypoglycemia, developmental delay, bradycardia. Therefore, the use of Biol during this period, as well as during breastfeeding, is not recommended.
Biol is taken with caution:
    Hyperthyroidism;
    AV blockade I degree;
    Congenital heart disease;
    Depression;
    Chronic heart failure with myocardial infarction within the last 3 months;
    Severe renal failure and impaired liver function;
    Conducting desensitizing therapy;
    Type 1 diabetes mellitus;
    Pheochromocytoma;
    Prinzmetal's angina;
    Psoriasis;
    Restrictive cardiomyopathy.

Method of administration and dosage

It is recommended to take Biol on an empty stomach in the morning.
In the treatment of arterial hypertension and coronary heart disease, 5 mg is usually taken once a day. If necessary, the dose can be doubled, but not more than 20 mg per day.
In chronic heart failure, after the obligatory special titration phase under regular medical supervision, treatment begins with 1/4 tablet of 5 mg Biol. If the dynamics are positive, then gradually, depending on individual tolerance, the dose can be increased to 5 or 10 mg per day (the maximum daily dose is 10 mg).
It should be borne in mind that at the beginning of treatment, arterial hypotension or bradycardia may develop. In this case, it is recommended to select the necessary concomitant standard therapy, as well as temporarily reduce the dose taken.
The presence of kidney or liver disease usually requires a reduction in the dosage of the drug; the elderly do not need such an adjustment.

Side effects

Biol can most often cause bradycardia and palpitations.
In addition, the drug can cause:
    A pronounced decrease in blood pressure;
    Increased peripheral circulatory disorders and a feeling of coldness in the extremities;
    Dizziness and headache;
    Asthenia;
    Increased fatigue and sleep disturbances;
    Depression and anxiety;
    Nausea, vomiting, dryness of the oral mucosa, diarrhea or constipation, abdominal pain.
Much less often, the use of Biol causes swelling of the ankles and feet, increased frequency of angina attacks, impaired AV conduction, arrhythmia, chest pain, orthostatic hypotension, shortness of breath, increased blood pressure.
In rare cases, possible: confusion or short-term memory loss, back pain, hallucinations, pruritus, nightmares, myasthenia gravis, tremors, psoriasis-like skin reactions, conjunctivitis and muscle cramps.
In case of an overdose of Biol, the following can be observed: severe bradycardia, arrhythmia, fainting, marked decrease in blood pressure, dizziness, ventricular extrasystole, AV blockade, acrocyanosis, acute heart failure, hypoglycemia, bronchospasm, difficulty breathing and convulsions.

Special instructions

With the simultaneous use of Biol with antiarrhythmic drugs of class 1 and 3, beta-blockers for topical use (including drops for the treatment of glaucoma), beta-adrenomimetics, mefloquine, insulin, ethanol, nifedipine, diuretics, clonidine, sympatholytics, tricyclic and tetracyclic antidepressants, tricyclic and tetracyclic antidepressants , antipsychotic, sedative and hypnotic drugs can develop undesirable side effects, leading to a change in the therapeutic effect of the use of drugs.
If depression develops, drug therapy is recommended to be discontinued.
Treatment should not be interrupted abruptly, since there is a great risk of developing myocardial infarction and severe arrhythmias. Cancellation is carried out gradually, reducing the dose for two weeks or more, every 3-4 days by one quarter.
It is necessary to stop taking Biol before donating blood and urine for the study of the concentration of normetanephrine, vanillin mandelic acid, catecholamines, antinuclear antibodies titers in them.
The effectiveness of beta-blockers is reduced in patients who smoke.
In the process of Biol therapy, special care should be taken when driving vehicles and engaging in potentially hazardous activities that require increased concentration of attention and high speed of psychomotor reactions.
