Nasarel is a drug for the treatment of allergic rhinitis.
The drug is available in dispenser bottles and is used for intranasal irrigation of the nasal concha mucosa. It is used in allergic rhinitis, when a person is worried about congestion and wear, tearing, sneezing and other symptoms of hypersensitivity.
The main active ingredient of this drug is fluticasone.

Release form and composition

Nasarel is available in the form of a white nasal spray. The suspension is packaged in dark glass bottles with a dispenser and a protective cap.
    The active ingredient is overdone fluticasone propionate. It is a local corticosteroid. Fluticasone inhibits the division of mast cells and eosinophils, and also inhibits the release of inflammatory mediators. The effect occurs within 5-10 minutes after the first application and lasts up to 24 hours.
In addition, Nasarel contains excipients: cellulose, carmellose, dextrose, phenylethanol and polysorbate.

Pharmacological effect

It is a localized glucocorticoid that, when used at recommended dosages, relieves inflammation and excessive swelling, exhibits antihistamine properties. Anti-inflammatory effect is achieved through the interaction of the main active ingredient of the drug with special receptors of the corticosteroids.
Fluticasone is able to inhibit the proliferation of lymphocytes, neutrophils, eosinophils, macrophages, as well as mast cells. At the same time, there is a decrease in the production and release of inflammatory mediators, as well as other biologically active components (for example, PG, cytokines, RT, guitar) during the early or late stages of allergy.
Antiallergic action is observed after 2-4 hours from the time of use of the spray. This significantly reduces the feeling of itching in the nasal cavity, eliminates the manifestation of rhinitis and sneezing, the nasal congestion disappears, the patient's general condition is facilitated. The use of glucocorticoids reduces the tearing of the eyes associated with allergic rhinitis. The duration of the nasal spray is maintained for 24 hours after irrigation of the nasal passages.
The use of drugs in therapeutic dosages does not have a systemic effect on the body, there is a minimal effect on the work of the hypothalamus, pituitary, and adrenal glands.

Indications for use

The instruction recommends taking Nasarel for prophylactic purposes, as well as for the treatment of a variety of allergic rhinitis (rhinitis). The drug can be used for the treatment of seasonal and year-round allergic reactions.
It is also allowed the use of this glucocorticosteroid in the complex treatment of sinusitis (sinusitis, frontitis, ethmoiditis and sphenoiditis) with allergic etiology.
The medicinal effect has a anti-edema and antiallergic effect on the patient's body.

Contraindications

Use of Nasarel is absolutely contraindicated:
    children up to 4 years;
    in patients with personal hypersensitivity to fluticasone or minor spray ingredients.
The use of Nasarel with the utmost care is possible with:
    pregnancy;
    breastfeeding;
    nasal injuries;
    concomitant herpes;
    ulcerative lesions of the nasal mucosa;
    surgical interventions in relation to the nasal cavities, as well as in the postoperative period;
    infections of the respiratory tract of bacterial etiology (such therapy requires the addition of antivirals and / or antibiotics);
    parallel to the therapy with other glucocorticoids (including tablet forms, ointments, sprays, creams, nasal drops and anti-asthma inhalers).

Use during pregnancy and lactation

In pregnancy, Nasarel is not recommended for use, as it is a hormonal agent. If there is a need to use the drug, then before prescribing it, the doctor should weigh all the positive and negative aspects of the treatment with Nasarel. Fluticasone-based medicinal spray therapy is acceptable if the intended benefit to the mother outweighs the possible risk to the fetus.
This is the standard wording present in the annotation to the drug, because tests aimed at studying the harm of medicines are not carried out on pregnant and lactating women. Therefore, reliable information about whether the active ingredient penetrates into the children's organism does not exist.
Spray Nasarel penetrates in small quantities into breast milk. At the time of treatment they should temporarily stop lactation in order to avoid harm to the child.

Dosage and method of use

The instructions for use indicate that Nasarel is used intranasally.
    Adults and children 12 years and older are prescribed 2 doses (100 µg) in each nasal passage 1 time / day, preferably in the morning. In some cases, it is necessary to inject 2 doses into each nasal passage 2 times / day (the maximum daily dose is 400 μg). After reaching the therapeutic effect, you can enter a maintenance dose of 50 µg / day in each nasal passage (100 µg). The maximum daily dose should not exceed 400 micrograms (4 doses in each nasal passage).
    Children between the ages of 4 and 12 are prescribed 1 dose (50 μg) 1 time per day in each nasal passage, preferably in the morning. The maximum daily dose should not exceed 200 mcg in each nasal passage. It is necessary to apply the minimum dosage to ensure effective elimination of symptoms.
    Elderly patients do not require dose adjustment.
To achieve the full therapeutic effect of the drug should be used regularly.

Terms of use of the drug

The nasal spray bottle is equipped with a protective cap that protects the tip from dust and contamination.
When first applied, it is necessary to prepare the bottle by pressing the dispenser 6 times. Spray mechanism unlocks. If the drug has not been used for more than one week, prepare the vial again and unlock the spray mechanism.
Next you need to do the following manipulations:
    clear nose;
    insert the vial tip into the nasal passage, closing the other nasal passage;
    tilt your head slightly forward while continuing to hold the aerosol bottle vertically;
    starting to inhale through the nose, press once on the spray mechanism;
    exhale through the mouth.
In another nasal passage, the drug is administered in a similar way.
After use, blot the tip with a clean cloth or handkerchief and cover it with a cap. Spray should be washed at least 1 time per week. To do this, remove the tip, rinse it in warm water, dry it and then carefully set it in the upper part of the bottle. Wear protective cap.
When clogged holes should be removed tip, immersed in warm water and leave for a certain time. After that, the tip must be rinsed with running water, dried and re-put on the bottle. The hole can not be cleaned with a pin or other sharp object.
After opening the package, the preparation can be used until the expiration date.

Side effects

When taking this glucocorticosteroid, patients sometimes have local and allergic negative side effects.
Local include:
    dryness and burning in the nasopharynx;
    nasal congestion;
    nosebleeds;
    headaches;
    perforation of the nasal septum.
Allergies include:
    anaphylactic reactions;
    swelling of tissues of the face and nasopharynx;
    rash on the skin;
    bronchospasm.
Also, with prolonged use, there may be a disorder in the functioning of the adrenal cortex and some others.

Overdose

Symptoms of chronic and acute overdose are not registered.
In the case of a study involving volunteers, intranasal administration of the drug (2 times a day, 2 mg of fluticasone for 7 days) did not show any effect on the work of the hypothalamic-pituitary-adrenal system.

special instructions

Before you start using the drug, read the specific instructions:
    With the appointment of GCS for intranasal use in high doses for a long time increases the risk of systemic effects of GCS. With prolonged use of the drug Nasarel requires regular monitoring of the function of the adrenal cortex.
    Simultaneous use with CYP3A4 isoenzyme inhibitors (ritonavir, ketonazole) requires careful monitoring of patients, since these drugs can cause an increase in plasma fluticasone propionate concentration.
    In the treatment of seasonal allergic rhinitis, Nasarel is quite effective, however, in the case of particularly high concentrations of allergens in the air during the summer, additional treatment may be required.
    Because GCS for intranasal use, even when used in therapeutic doses, can cause a slowdown in the growth of children with long-term therapy, it is necessary to regularly monitor the growth of the child and to adjust the dose of the Nasarel drug in a timely manner.
    When prescribing the drug, Nasarel patients with tuberculosis, an infectious process, herpetic keratitis, as well as those who have recently undergone surgery on the mouth and nose, should carefully evaluate the ratio of the possible risk and the expected benefit.

Drug interactions

When using the drug, you must consider the interaction with other drugs:
    Interaction with other drugs is unlikely, since with intranasal administration the concentrations of fluticasone in plasma are very low.
    With simultaneous use with other inhibitors of the cytochrome P450 system (erythromycin, ketoconazole), there is a slight increase in the concentration of fluticasone propionate in the blood, which has virtually no effect on cortisol content.
    With simultaneous use with strong inhibitors of CYP3A4 isoenzyme (ritonavir), it is possible to enhance the systemic action of fluticasone and the development of side effects (Cushing syndrome, suppression of the function of the adrenal cortex).

Reviews

We picked up some reviews of people taking the drug Nasarel:
Nadia
    I suffer from allergic rhinitis, which is manifested due to dust. I tried many medicines, even the vaunted Nasarel, but he did not suit me. I started having headaches, and the nasal mucosa began to dry up terribly. I had to cancel the medication prescribed by the doctor and switch to a safer, non-hormonal one. My review - I do not advise anyone.
Tasha
    In case of vasomotor rhinitis, only Nasarel helps me and nothing else. I dream to do without it somehow, but it's hard to live with a swollen nose, and every winter I buy ...
Olga
    Almost month I use Nasarel. Nose as not breathing, and not breathing !!! The diagnosis is chronic allergic rhinitis, although I don’t know where it comes from, if after the acute respiratory viral infection the nose has stopped breathing !!!

    After a severe cold, I could not get rid of a cold. He interfered with normal sleep and work, my head ached. After going to a specialist, I received a prescription for Nasarel in the form of a spray. The doctor warned that the tool is strong, so you should use it with caution.
