To effectively combat allergies and its manifestations, a variety of drugs have been developed, the therapeutic effect of which is to block histamine H1 receptors. The second-generation antiallergic drug "Kestine" (international non-proprietary name - Ebastine) belongs to the middle price category.

Pharmacological action and pharmacokinetics

Kestine conducts a competitive blockade of the histamine neurotransmitter H1 receptors, resulting in reduced vascular permeability and the risk of bronchial smooth muscle spasm. The drug has a long-lasting effect, which manifests itself an hour after administration and lasts two days.
    Absorption occurs immediately after ingestion. In the liver, the active ingredient is converted to the metabolite karebastin. You can accelerate the absorption and formation of karebastin with the help of fatty foods.
During treatment with Kestine, there is a gradual cumulation of karebastin in the body, therefore, at the end of the course, the therapeutic effect persists for six more days. With continuous use of the drug for three days, the equilibrium concentration of the metabolite is established within 130–160 ng / ml. The drug has no sedative effect.
The tool is excreted by the kidneys. Half of the pharmacological effect is lost after 15–19 hours; with hepatic and renal failure, this figure can reach 27 and 23-26 hours, respectively.

Release form

"Kestine" is presented in the following forms:
    Tablets are white or almost white in a film coat. The concentration of the active substance in the tablet is 10 or 20 mg. Ingredients: Ebastine, cellulose, starch, magnesium stearate, lactose monohydrate, croscarmellose sodium, titanium dioxide. Tablets are engraved "E10" or "E20" on one side.
    Tablets lyophilized (instant) for resorption. Ingredients: Ebastine (10 mg), gelatin, mannitol, sweetener aspartame, mint flavor.
    Transparent syrup with anise smell. Supplied in a dark glass bottle with an aluminum cap, a measuring syringe is also attached. The composition of the syrup: Ebastine, glycerin, sorbitol solution, glycerin oxystearate, lactic acid, sodium methyl parahydroxybenzoate, water, other substances in a concentration of less than 1 mg, acidity regulator.

Indications for use

"Kestine" is assigned under the following conditions:
    allergic rhinitis of various origins,
    allergic conjunctivitis,
    urticaria of various genesis,
    Quincke's edema
    other diseases caused by a high concentration of histamine.

Contraindications

It is necessary to refuse reception of "Kestine" at high sensitivity to its components, and also during pregnancy. Tests on animals confirmed the safety of ebastine for the fetus, but data on this kind of effect on the human body today does not exist. It is also unknown whether ebastine and its metabolite karebastin are excreted into breast milk, so the drug should not be consumed by nursing mothers.
    The drug is contraindicated in people with lactase deficiency, lactose intolerance and glucose-galactose malabsorption.
Some forms of "Kestine" (lozenges in the mouth) are not prescribed in violation of the protein metabolism. Lyophilized tablets are contraindicated in children and adolescents under 15 years old, tablets in the shell - children under 12 years of age. Use of the drug is not recommended for children under six.
    Patients with hypokalemia, hepatic or renal insufficiency, and an increased QT interval should take Kestine with caution.


Side effects

At therapeutic doses, the drug causes few side effects. Sometimes its use leads to drowsiness, headache and dry mouth. Isolated cases of sinusitis and rhinitis, as well as nausea, abdominal pain and sleep disturbance.
    The drug does not adversely affect the central nervous system, so patients are allowed to engage in activities that require high concentration: for example, driving a car.
An overdose, in which the patient feels fatigue and dry mouth, can only be caused by the simultaneous reception of at least a thirty-fold dose of Kestine. Since there is no drug that could weaken or neutralize the effect of Ebastine, in case of overdose, it is necessary to clear the stomach and treat individual symptoms.
It is recommended to conduct skin tests for allergies one week after the end of taking Kestine, as it may affect the results.

Mode of application

Kestine is taken orally once a day. Meal time does not matter. The drug is not required to drink water. Dosage varies for children and adults:
    adults over 15 years old - 10–20 mg, depending on the severity of the disease,
    adolescents from 12 to 15 years old - 10 mg or 10 ml (syrup),
    children from six to 12 years old - 5 ml (syrup).
The maximum daily dose of the drug for the treatment of patients with impaired liver - 10 mg.
Lyophilized tablets are designed for resorption in the mouth. They do not have a shell and are generally quite fragile, so they should be taken out of the blister gently, without pressing.

Interaction with other drugs

"Kestine" is not recommended to be taken in combination with the following drugs:
    fluconazole, ketoconazole and other antifungal agents,
    erythromycin, roxithromycin and other macrolides (increase the risk of increasing the QT interval).

    indirect anticoagulants,
    theophylline,
    cimetidine
