pharmachologic effect
Antiallergic drug. Loratadin belongs to the group of competitive histamine antagonists, selectively blocks peripheral histamine H1 receptors. Loratadin affects the histamine-dependent stage of allergic reactions, as well as reduces the migration of eosinophils, limits the release of inflammatory mediators at the cellular stage of allergic reactions. Prevents the development and facilitates the course of allergic reactions, has anti-exudative and antipruritic action, practically does not have anticholinergic and antiserotonin action.
Does not penetrate the BBB, does not affect the central nervous system, does not cause addiction.
Antiallergic effect develops after 30 minutes and lasts 24 hours.
Pharmacokinetics
Suction
Quickly and completely absorbed from the digestive tract. The time to reach Cmax is 1.3-2.5 hours. Eating slows down the time to reach Cmax by 1 hour.
Distribution
Plasma protein binding - 97%. Css loratadine and its metabolite is achieved on the 5th day of administration. Does not penetrate the BBB.
Metabolism
Metabolized in the liver to form the active metabolite of descarboethoxyloratadine with the participation of CYP3A4 enzymes and, to a lesser extent, CYP2D6.
Removal
T1 / 2 loratadine - 3-20 hours (average 8.4 hours), active metabolite - 8.8-92 (average 28 hours).
Excreted in urine and bile.
Pharmacokinetics in special clinical situations
Cmax increases in elderly patients by 50%, T1 / 2 loratadine is 6.7-37 h (average 18.2 h), the active metabolite - 11-38 (average 17.5 h).
With alcoholic liver damage, Cmax and T1 / 2 increase in accordance with an increase in the severity of the disease.
In patients with chronic renal failure and during hemodialysis, the pharmacokinetics are virtually unchanged.
Dosage of the drug Clarisens

Drug interactions
Ethanol reduces the effectiveness of loratadine.
Erythromycin, cimetidine, ketoconazole increase the concentration of loratadine in the blood plasma, without causing clinical manifestations and without affecting the ECG.
Inductors of microsomal oxidation (phenytoin, barbiturates, zixorin, rifampicin, phenylbutazone, tricyclic antidepressants) reduce the effectiveness of loratadine.
The use of clarisens during pregnancy
The drug is contraindicated during pregnancy and during breastfeeding.
Use in childhood

Clarisens - side effects
