Gaynomax is a combined agent for topical use in gynecology, which has antifungal, antibacterial and antiprotozoal effect.

Release form and composition

Dosage form - vaginal suppositories: flat shape with a rounded end in the form of a truncated ellipse, yellowish (7 pcs. In contoured cell-free packages, in a cardboard box 1 package complete with 7 disposable fingertips).
Active ingredients in each suppository:
    tinidazole - 150 mg;
    thioconazole - 100 mg.
Additional substance: witepsol.

Pharmacodynamics

The action of Gaynomax is determined by the properties of its active substances.
Thioconazole is a synthetic antifungal substance with high activity against yeast-like and other fungi (dermatophytes), as well as against bacterial pathogens - Bacteroides spp., Gardnerella vaginalis, some gram-positive bacteria (including Staphylococcus and Streptococcus spp.).
The mechanism of action of thioconazole causes an increase in the permeability of the cell membrane of the fungus by inhibiting the synthesis of ergosterol (which is its main component). With an increase in the permeability of the cell membrane, the intracellular ionic balance is disturbed, which causes cell death. According to clinical studies, the efficacy of thioconazole in terms of clinical and microbiological parameters is higher than miconazole.
The second active component of Gaynomax, tinidazole, is active against most anaerobic bacteria and protozoal pathogens (Trichomonas vaginalis, Giardia lamblia, Entamoeba histolytica, Bacteroides fragilis, Gardnerella vaginalis, Bacteroides spp., Bacterium spp., Bacterella spp., Bacterella spp. spp., Peptostreptococcus spp.).
The mechanism of action of tinidazole is not fully understood. The reduction of the nitro group is mediated by the low redox potential and the ferredoxin system.

Absorption of the active components of Gaynomax with intravaginal use is negligible.


Indications for use

According to the instructions, Gaynomax is a drug for the treatment of vaginitis caused by pathogens of the genera Gardnerella, Candida or Trichomonas (gardnerella vaginitis, Trichomonas vulvovaginitis, vaginal candidiasis, nonspecific vaginitis).
Also, the tool is used for mixed infections.

Contraindications

    leukemia, hematopoietic disorders;
    organic diseases of the nervous system, including in the anamnesis;
    virginity;
    age under 18;
    I trimester of pregnancy;
    lactation;
    hypersensitivity to the components of the drug, derivatives of tinidazole or thioconazole.

Instructions for the use of Gaynomax: method and dosage

Gaynomax suppositories are intended for intravaginal use. Suppositories should be inserted deep into the vagina using the supplied disposable fingertips.
Adult women are prescribed 1 suppository 1 time per day at night.
The course of treatment is 7 days.
It is also possible to carry out an alternative treatment regimen: 1 suppository 2 times a day (morning and evening) for 3 days.

Side effects

Side effects identified in clinical trials (the results of both treatment regimens were evaluated - 3 and 7 days; 348 women participated in the studies):
    3.3-4.5% - allergic reactions;
    1.9% - local itching;
    1.7% - burning sensation.
In most cases, adverse reactions were moderate. In only 0.75% of cases, treatment had to be interrupted due to allergic reactions.
Side effects identified during post-marketing observations:
    local reactions: rarely - irritation and swelling of the vaginal mucosa;
    central and peripheral nervous system: rarely - increased fatigue, headache;
    urinary system: rarely - darkening of urine;
    allergic reactions: rarely - skin rashes; in individual cases - Quincke's edema.
Although no cases of transient leukopenia were noted with intravaginal use of a combination of tinidazole and thioconazole, it should be borne in mind that there are cases of development of this symptom associated with gastrointestinal or neurological symptoms due to the use of tinidazole.

Overdose

In case of accidental oral intake of a large amount of Gaynomax, gastric lavage is performed. There is no specific antidote, symptomatic therapy is indicated.


Special instructions

During treatment and within 2 days after discontinuation of the drug, it is recommended to refrain from drinking alcohol.
During the period of Gaynomax therapy, you should not use mechanical contraceptives - condoms, contraceptive diaphragms or caps, which is due to the risk of interaction of Witepsol with the latex base of these funds.
The drug does not have any effect on the speed of reactions and the ability to concentrate.

Application during pregnancy and lactation

    I trimester of pregnancy, lactation period: therapy is contraindicated;
    II and III trimesters of pregnancy: Gaynomax suppositories can be used after a doctor evaluates the balance of benefits with a possible risk.

Childhood use

Gaynomax therapy is not prescribed for patients under 18 years of age, as well as for virgins.

Drug interactions

Tinidazole in a dosage form for systemic action, used simultaneously with ethanol, can cause skin redness, nausea, intestinal colic, and vomiting. When using vaginal suppositories, tinidazole practically does not enter the systemic circulation, but the likelihood of interaction with ethanol should be taken into account.
