Crinone is a progestogen drug that causes the transition of the uterine mucosa into the secretory phase from the proliferation phase due to the action of follicle-stimulating hormone (FSH), which reduces the excitability and contractility of the fallopian tubes and uterine muscles.

Release form and composition

Dosage form - vaginal gel: soft consistency, homogeneous, white or almost white, has a specific odor (1 dose in disposable polyethylene vaginal applicators, 6 or 15 applicators in a carton pack).
Active substance: progesterone, 1 applicator (1.125 g) contains - 90 mg.
Additional substances: purified water, hydrogenated palm oil glyceride, light liquid paraffin, carbomer 974R, polycarbophil, sorbic acid, sodium hydroxide, glycerol.

Pharmacodynamics

The active ingredient of Crinone is synthetic progesterone, which is completely identical to natural progesterone, the hormone of the corpus luteum. Under its influence, there is a transition of the uterine mucosa from the proliferative phase, for which the follicle-stimulating hormone (FSH) is responsible, to the secretory phase. The substance reduces the contractility and excitability of the muscles of the uterus and fallopian tubes.
Progesterone inhibits the work of the hypothalamic factors for the release of FSH and LH (luteinizing hormone), suppresses ovulation and prevents the synthesis of gonadotropic hormones in the pituitary gland.
Crinone contains progesterone as a vaginal gel placed in a polymeric delivery system that binds to the vaginal mucosa and provides a constant release of the drug for at least 3 days.

Pharmacokinetics

When using Crinone at a dose of 90 mg, the maximum concentration of progesterone in the blood (11 ng / ml) is reached after about 6 hours. The elimination half-life is 34-48 hours.
Most of the progesterone is metabolized in the liver. Vaginal application significantly reduces the effect of "first pass" through the liver. The main metabolite is 3-α, 5-β-pregnandiol excreted in the urine.

Special instructions

Before prescribing Crinone, the patient should undergo an examination by a doctor, including an analysis of a cervical smear (Pap test), an assessment of the development and condition of the pelvic organs and mammary glands.
The drug can be used in the first trimester of pregnancy with insufficient function of the corpus luteum. At a later date, the appointment of the drug is not recommended.
Patients with any risk factors for the development of thrombotic disorders (pulmonary embolism, cerebrovascular accident, thrombophlebitis, retinal thrombosis) should be carefully monitored.
During treatment, special attention should be paid to possible early signs of thrombotic disorders. In the event of the appearance of any symptom that makes it possible to suspect the presence of such a violation, it is necessary to immediately cancel Crinone.
With long-term treatment, regular gynecological examinations are indicated to exclude the possibility of developing endometrial hyperplasia.
To avoid the risk of incomplete abortion when using the drug, you should regularly conduct an ultrasound examination or determine the level of chorionic gonadotropin.
In cases of irregular vaginal bleeding, including breakthrough bleeding, organic pathology must be ruled out.
In case of vaginal bleeding of unknown origin, an appropriate examination should be carried out.
Special monitoring during the treatment period is required for patients with a history of depression. If depressive symptoms appear, the use of Crinone should be discontinued.
In some cases, etrogen-progestogen drugs can reduce glucose tolerance, so patients with diabetes should be under close medical supervision.
Sorbic acid, which is part of the gel Crinone, can cause contact dermatitis.
The drug can cause drowsiness and fatigue, so you should be careful when driving and performing potentially hazardous activities (alcohol can enhance this effect).

Use during pregnancy and lactation

It is allowed to use Crinone in the first trimester of pregnancy with insufficient function of the corpus luteum. The appointment of the drug at later stages of pregnancy is not recommended.

For impaired renal function

In patients with kidney dysfunction, the drug is used with extreme caution and under the supervision of a physician due to the risk of fluid retention in the body.

For impaired liver function

According to the instructions, Crinone is prescribed with caution for liver dysfunctions.

Drug interaction

It is not recommended to use Crinone simultaneously with other intravaginal agents.

Reviews about Crinone

According to reviews, Crinone is necessary when planning pregnancy, since progesterone is responsible for preparing the endometrium for implantation of the fetal egg. Most patients claim that the vaginal gel is very convenient and easy to use and prefer this method of administration to intramuscular. Women also note that the bioadhesive gel is practically not subject to leakage, which minimizes the loss of the active substance compared to conventional progesterone suppositories.
Reviews about the drug as an element of assisted reproductive technologies are very common, which provides progesterone support for pregnancy up to 8–12 weeks. At 14 weeks, the placenta is already fully formed, so it is not advisable to use Crinone. A blood test for hCG is carried out starting from the 12th day after the transfer of the embryo into the uterus.
The appointment of Crinone can lead to an increase in body temperature up to 37.5 ° C, which is considered normal. During therapy, some women experience pinkish, beige or brown discharge. White discharge can be considered as a variant of the norm, however, their staining indicates the need for increased hormonal support for pregnancy and requires immediate medical attention.
With prolonged use, the base of the gel accumulates in the body. Therefore, experts recommend mechanical cleaning of the vaginal mucosa once every 2 days using a sterile gauze swab. This must be done before the next introduction of Crinone. Some patients complain of side effects such as constipation, drowsiness and headache, as well as the high cost of the drug.
