Release form and composition

Dosage form - oral solution: a clear, colorless or almost colorless liquid with a specific odor (50 and 100 ml each in brown / orange glass bottles, sealed with dropper caps and screw-on caps with first opening control, in a cardboard box 1 bottle with instructions for use of Levocarnil and with a measuring cup or without it).
Composition of 1 ml of solution:
    active substance: levocarnitine - 300 mg;
    auxiliary components: purified water, methyl parahydroxybenzoate, propyl parahydroxybenzoate, citric acid monohydrate.

Pharmacodynamics

L-carnitine is a naturally occurring substance related to B vitamins.
It participates in metabolic processes as a carrier of long chain fatty acids (including palmitic) from the cytoplasm to mitochondria, where they undergo a β-oxidation process with the formation of acetyl-CoA and adenosine triphosphoric acid.
It increases the secretion and enzymatic activity of gastric and intestinal juices, improves the absorption of food, fat and protein metabolism, reduces the fat content in muscles, and reduces overweight.
Reduces the degree of lactic acidosis, inhibits anaerobic glycolysis and the formation of keto acids. Promotes economical consumption of glycogen, increases its reserves in muscles and liver. Increases resistance to physical stress.
It has a lipolytic and anabolic effect. Improves tissue energy and metabolism.
With hyperthyroidism, it normalizes increased basal metabolism: it is a peripheral antagonist of the action of thyroid hormones, and not a direct inhibitor of thyroid function.

Pharmacokinetics

After oral administration, levocarnitine is well absorbed in the gastrointestinal tract. The maximum concentration is reached within 3 hours and remains in the therapeutic range for 9 hours. The drug easily penetrates the myocardium and liver, and more slowly into the muscles.
It is excreted from the body by the kidneys, mainly in the form of acyl ethers.

Indications for use

Levocarnil is used as part of complex therapy in the following cases:
    diseases accompanied by a lack of carnitine or its intense loss, such as mitochondrial diseases, cardiomyopathies, myopathies, hereditary diseases with concomitant mitochondrial insufficiency (to compensate for carnitine deficiency);
    anorexia nervosa syndrome;
    neurological manifestations in vascular, traumatic and toxic brain lesions;
    mild hyperthyroidism;
    skin diseases such as focal scleroderma, seborrheic eczema, psoriasis, discoid lupus erythematosus;
    chronic gastritis, chronic pancreatitis with reduced secretory function;
    rehabilitation period after diseases, injuries and surgical interventions (including to accelerate tissue regeneration);
    psycho-emotional stress, including in the elderly, and intense physical activity (to reduce fatigue, increase stamina and performance).
In addition, Levocarnil is used in adults for intensive training and in sports medicine (during the training process when practicing aerobic performance) in order to reduce body fat mass, increase muscle mass, improve coordination of movements and speed-strength indicators, accelerate muscle regeneration in traumatic injuries and for the prevention of post-training syndrome (accelerating recovery processes after physical exertion), as well as in pediatric practice when nursing a newborn those who have suffered asphyxia or birth injury, and premature babies, low weight gain and a sluggish sucking reflex, decreased muscle tone, insufficient development of mental and motor functions (including the prevention of these disorders in children at risk), lack of body weight and stunted growth in children under 16 years old.


Contraindications

The solution of Levocarnil is contraindicated in the case of individual intolerance to its components.
It is not recommended to use the drug during pregnancy and lactation.

Instructions for use: method and dosage

Levocarnil is used as part of complex therapy. The solution should be taken orally after dilution with liquid, 30 minutes before a meal. To improve the taste, the drug can be added to sweet drinks (compote, juice, jelly).
For ease of dosing, you can use the supplied measuring cup, medical syringe or pipette.
Recommended dosage regimens of Levocarnil depending on indications:
    diseases accompanied by a lack of carnitine (primary and secondary carnitine deficiency): 50–100 mg / kg (2–5 drops / kg body weight) 2-3 times a day for a course of 3-4 months;
    Anorexia nervosa syndrome, rehabilitation period after diseases, injuries and surgical interventions: 1500 mg (5 ml) 2 times a day for 1-2 months;
    neurological manifestations in case of vascular, traumatic and toxic brain lesions: 750 mg (2.5 ml) per day for 3-5 days. If necessary, the course is repeated after a break of 12-14 days;
    mild hyperthyroidism: 250 mg (13 drops) 2-3 times a day for 20 days with a repeat of the course after 1-2 months or continuously for 3 months;
    skin diseases: 750 mg (2.5 ml) in a course of 2-4 weeks;
    chronic gastritis, chronic pancreatitis with reduced secretory function: 375 mg (1.25 ml) 2 times a day for 1-1.5 months;
    long-term psycho-emotional and physical stress: depending on the severity of the condition, the dose of Levocarnil can vary from 750 mg (2.5 ml) 3 times a day to 2250 mg (7.5 ml) 2-3 times a day;
    intensive training and sports medicine: 2500 mg (8.33 ml) 1-3 times a day. When Levocarnil is prescribed for therapeutic purposes, the daily dose is 70–100 mg / kg (5000–7500 mg per day). The duration of the intake in the pre-competition period is 3-4 weeks, during the training process - up to 6-8 weeks.
Dosing Levocarnil for children:
    up to 3 years: the dose is determined by the doctor individually;
    3–6 years: 100 mg (5 drops) 2-3 times a day. The course of therapy is 1 month;
    6-12 years: 200-300 mg 2-3 times a day for at least 1 month;
    with growth retardation: 250 mg (13 drops) 2-3 times a day for 20 days with a repeat of the course after 1-2 months or for 3 months continuously.


Side effects

    from the gastrointestinal tract: dyspeptic disorders, gastralgia;
    allergic reactions;
    others: myasthenia gravis (in patients with uremia).

Overdose

In case of an overdose of Levocarnil, dyspeptic disorders are observed, in patients with uremia - myasthenic disorders.
It is recommended to do a gastric lavage and take activated charcoal.

Special instructions

Treatment of children under 3 years of age should be carried out under the close supervision of the attending physician.
Influence on the ability to drive vehicles and complex mechanisms
Levocarnil does not violate the concentration of attention and speed of reactions, therefore, does not affect the ability to drive vehicles and perform potentially dangerous activities that require special attention and speed of reactions.
