Production form, composition

The medicine is in the form of a white suspension with a lemon aroma. When preserving the drug, it is possible to separate the liquid without color. With vigorous stirring of the vial, the suspension regains its uniformity.
Active substances: aluminum hydroxide gel, magnesium hydroxide paste, benzocaine.
Inactive component: sorbitol, hydroxyethyl cellulose, para-hydroxybenzoic acid methyl ether, propyl ester of para-hydroxybenzoic acid, butyl parahydroxybenzoate, saccharin, lemon oil, purified water.

Pharmacodynamics

Almagel A is a pharmaceutical product that combines a combination of aluminum hydroxide and magnesium hydroxide. The drug neutralizes free hydrochloric acid, inhibiting the effect of pepsin, which entails a decrease in the digesting power of gastric juice. It protects against irritation due to the formation of colloidal solutions covering the mucous membrane, due to which it helps to remove toxic substances from the stomach cavity. Protects the gastric mucosa by stimulating the production of prostaglandins.
This helps to protect the mucous membrane from the formation of erosion and inflammatory processes due to exposure to irritating substances and provoking erosive lesions.
The therapeutic effect is manifested after 3 - 5 minutes after application.
The duration of the effect of the pharmaceutical is based on the speed of the emptying of the stomach. When treating an empty stomach, the effect of the medication lasts approximately 60 minutes. When using Almagel A one hour after eating, the effect of the medication can last about 3 hours.
It does not provoke a repeated increase in gastric secretion and acidity of gastric juice.
Aluminum hydroxide inhibits the production of the main proteolytic enzyme of the digestive tract, neutralizes hydrochloric acid, creating aluminum chloride, which in an alkaline environment is converted into alkaline aluminum salts.
Magnesium hydroxide similarly neutralizes hydrochloric acid, transforming into magnesium chloride. Therefore, there is an obstruction of the effects of algeldrat, which provokes delayed, difficult or systematically insufficient defecation.
Benzocaine promotes effective and prolonged analgesia for severe pain attacks.
Sorbitol, located in Almagel A, enhances the secretion of bile and has a slight laxative effect, enhancing the effects of magnesium hydroxide.
The pharmaceutical preparation does not raise the pH sharply in the stomach, but sets its indications for physiological norms between its use. The medication creates a protective film that allows active substances to be distributed on the gastric mucosa, exhibiting a long-term local effect, blocking the release of carbon dioxide in the stomach, which causes increased gas formation and pain in the stomach.

Pharmacokinetics

Almagel A is a medication that is not absorbed. Following the rules prescribed in the instructions and not exceeding the permissible doses, the duration of therapy, it is almost not absorbed in the digestive tract. The medicine exhibits a long-term effect without disturbing the water-salt balance and without causing an increase in the pH of the blood and other body tissues due to the accumulation of alkaline substances or other metabolic disorders. The medication does not cause irritation of the urinary tract and the formation of stones in the organs of urination.

Indications for use

Almagel A, according to the instructions for use, is used for treatment and for prevention. For therapeutic purposes, a medication is prescribed in the presence of such pathologies:
The acute inflammatory process of the gastric mucosa, which occurs with a short exposure to strong irritants.
Chronic disease of the gastric mucosa with increased and normal secretory function of the stomach.
    Inflammation of the duodenal mucosa.
Inflammatory disease of the mucous membrane of the small intestine with impaired function.
    Inflammation of the colon mucosa.
Peptic ulcer of the stomach and duodenum in the acute phase.
    Diaphragmatic hernia.
    Gastroesophageal reflux.
    Gastroesophageal reflux disease.
Acute or chronic focal destruction of the gastric mucosa caused by various factors.
Superficial defects of the mucous membrane of the stomach and duodenum.
    Pancreatic inflammatory disease.
Discomfort or sternal burning, spreading upward from the epigastric region along the esophagus.
Pain attacks in the epigastric region after a violation of the diet, excessive drinking of alcohol, abuse of nicotine, coffee, taking medications that contribute to irritation of the gastric mucosa.
For preventive purposes, Almagel A is prescribed to reduce the irritating effect of taken medications on the gastric mucosa.

Contraindications

A drug is not prescribed for such conditions and pathologies:
Excessive sensitivity to the active ingredients of Almagel A or other substances of the inactive component.
    The period of gestation.
    Children under 10 years old.
Severe pathological condition of the kidneys with concomitant functional disorders (there is a risk of higher levels of magnesium in the blood than normal and aluminum poisoning).
An incurable degenerative disease of the central nervous system, characterized by a gradual loss of mental abilities.
A decrease in the level of phosphates in the blood below the physiological norm.
    Congenital fructose malabsorption.

Pregnancy, lactation

In ongoing clinical experiments on animals, toxic effects on the fetus were not detected. But information on the use of pharmaceuticals during gestation and breastfeeding is not enough for women.
The use of Almagel A suspension is not recommended for women who have a fetus and are breast-feeding babies. If treatment with a medication in this period of life is necessary according to indications, then the medicine is taken under medical supervision and not longer than 5-6 days.

Regulations for use, dosage

With medical intentions, Almagel A is prescribed to adults and children after 15 years, 1 to 2 tablespoons up to 4 times a day. If necessary, increase the dose to 3 scoops.
For children 10 to 15 years of age, the medication is prescribed in a dosage as for adults, divided in half. The suspension is used 60 minutes after eating and before going to bed in the evening.
Having achieved the necessary therapeutic effect, the dose recommended for a day is reduced to 1 measured spoon (5 ml) up to 4 times a day. Therapy lasts 2 to 3 weeks.
After taking Almagel A, you can not take the liquid for 15 minutes.
Before applying Almagel A, the vial is shaken vigorously.
For preventive purposes, the suspension is taken before taking medications that irritate the gastric mucosa, for 15 minutes, 5-15 ml.

Adverse reactions

A pharmaceutical can provoke delayed, difficult or systematically insufficient defecation, which stops after a dosage reduction.
Such changes can rarely be observed:
Painful sensation in the epigastrium or pharynx, ending in vomiting.
Involuntary reflex emptying of the stomach through the mouth.
Painful attacks in the stomach due to pathological contractions of smooth muscles.
    Distortion of taste.
    Allergoses.
The increase in blood quantitative number of magnesium is higher than normal.
Long-term treatment with a medication in people with burdened pathological conditions of the kidneys with concomitant functional disorders and on dialysis can cause a disturbance in mood and mental activity.
High-dose and long-term therapy in patients with a lack of phosphorus in the body can cause a violation of bone mineralization.

Overdose

With a single dose increase, no consequences are noted, except for a delayed, difficult or systematically insufficient act of defecation, excessive occurrence and accumulation of gases in the intestine, a sensation of a metallic taste in the mouth.
Long-term use of the drug Almagel A in high doses can provoke the formation of urolithiasis, difficulty in bowel movement, drowsiness, and an increase in the level of magnesium in the blood above normal.
Symptoms of base buildup or excessive loss of acids from the body are also noted:
Changes in mental activity, emotional lability.
Numbness or soreness in the muscles.
    Inhibition of respiratory ability.
    Distortion of taste.
If these signs appear, the pharmaceutical preparations are immediately stopped and measures are taken to eliminate Almagel A residues from the body - they wash the stomach, induce vomiting and prescribe activated charcoal.

Drug Compatibility with Other Medicines

Almagel A is able to absorb some pharmaceuticals, reducing their absorption. Therefore, it is recommended to take other medications 2 hours later or after using an antacid medication.
With parallel treatment with medications that dissolve in the intestine, an increased pH of gastric juice can provoke a rapid dissolution of their membrane, which will lead to irritation of the stomach and duodenum.

Features of use

You can not use the pharmaceutical product in people with disorders of bowel movements, with pain attacks in the stomach of unknown etiology, and with an assumption of appendicitis, in the presence of a colon disease with immune inflammation of its mucous membrane. In a disease characterized by the formation of small saccular protrusions in the intestinal wall. The medicine is contraindicated during surgical intervention, when part of the colon is inserted through the abdominal wall to the outside or when the ileum loop is removed to the anterior abdominal wall to form a permanent or temporary fistula. Almagel A is not prescribed for prolonged diarrhea, with a pathological increase in hemorrhoidal nodes, with an upset of acid-base balance in the body, with cirrhosis of the liver, and severe functional disorders of the heart.
Long-term treatment with a medication for up to 20 days in people with impaired renal function requires the supervision of a doctor and control of the level of magnesium in the blood serum.
Suspension is approved for the treatment of patients with diabetes. But the content of sorbitol in the drug makes it inaccessible for the treatment of patients with congenital fructose malabsorption.
